An 6 month old infant was observed sleeping in a swing. This was corrected at inspection when the infant was moved to their crib bringing the operation into compliance.

It was determined through the course of the investigation that the infant room has not maintained proper ratio.

It was determined through video surveillance that a caregiver in the infant room was using their cell phone for personal use while supervising infants alone.

2 caregiver files reviewed, that have been employed longer than 90 days, did not have a current CPR certification.

2 infants in the infant room were not buckled into the kidney bean toddler table. This was corrected at inspection when the children were buckled in.

The operation had not conducted a fire drill since July 2025.This was corrected at inspection when the operation conducted a fire drill bringing the operation into compliance.

A caregiver in the 4 yr old room stated there were 18 children in the classroom when there were 17 children in the classroom.

It was determined through the course of the investigation that a caregiver was under the influence of alcohol while supervising children alone for approximately 1 hour.

It was determined through the course of the investigation that a caregiver consumed alcohol on the premises of the daycare.

The operation was lacking an updated annual health inspection.

A fire drill was not documented for the month of November 2022. This was corrected at inspection when the center conducted a fire drill during the inspection.

A child that has a food allergy to eggs was lacking an emergency allergy plan signed by both the parent and the physician. There is no documentation in the child's file that indicates the child is no longer allergic to eggs.

The loose-fill surfacing under the playground structure was not maintained at a depth of at least 6 inches.

Infant feeding instructions for infants not yet on table food were not updated in the 30 day required time frame.

A food allergy plan was not available for a child who has a doctor diagnosed food allergy to spinach.

In the infant room ages 10-17 months old did not have their hands washed prior to eating lunch.

A two year old child was left unattended on the playground for approximately a minute and a 1/2. He was unharmed and brought back inside by a caregiver to rejoin his group.

A child was left on the playground for a brief period of time and the parent was not notified until two and half hours later.

A child who was upset and crying in pain reached out to a caregiver for help and the caregiver pushed him away and made no effort to see why he was upset.

Two caregivers sitting in the classroom could see that a toddler was visibly upset and crying. The child cried and appeared to be in pain. The caregivers allowed him to cry out for a minute and a half before picking him up to assess what could be wrong.

A two year old child walked into a restroom and was unattended for approximately four minutes when the group was transitioning from the gym to the classroom.

Based on information obtained from a DFPS investigation, a caregiver was not properly supervising a child in care that resulted in a misdiagnosed injury and the child being sent home without the appropriate treatment.
