The operation did not conduct the following emergency practices: severe weather, shelter in place, and lock down drill for the 4th time in 2025 and the fire drill for the months of October, November, and December of 2025.

One out of the four staff records monitored did not have current first aid training.

Two out of four staff records monitored did not have the affidavit form 2912.

One out of the four staff records monitored did not have current CPR training.

Caregiver supervising a group of children was observed outside the classroom.

There was trash in the outside playground. In one of the restrooms, there was trash on the floor, next to the toilet paper.

There is a tree stump of approximately 4-7 inches in height in the playground. The surrounding area of the tree stump is uneven, making it a tripping hazard. The center's wall on the back of the building, on the side of the outdoor playground has a removable piece measuring 22" by 37." The piece was detached from the wall exposing the underground of the building to the children's playground. This posed a safety hazard as the area where the missing piece is located is within childrens' reach.

Diaper changing supplies were within reach in the pre-k, and toddler class.

Caregiver did not demonstrate good judgement when she cut a child's nails, and it resulted in child getting a cut in a finger.

Incident report was not provided to parent of a child who had its nails cut at the operation and was cut as a result.

The outdoor playground safety checklist was not available for review.

Three out of the six staff records reviewed, did not have the required annual safety trainings.

At arrival time, the three year old classroom was observed to have infants and children with more than a 18 month gap in age. The youngest child was observed to be 4 months old and the oldest was 4 years of age.

One medication monitored did not have the original container.

One out of the six children's records monitored did not have an immunization record.

In the 3 year old classroom children were observed to be running around in the room and the caregiver did not redirect behavior. 1 child observed slapping another child. Children observed climbing furniture and hiding underneath. Children dropped food and ate the food off the floor and one child was observed taking out an electric safety plug from an outlet and attempted to stick his finger inside. One child was observed feeding an infant child food that was on the floor.

Two out of the six staff records monitored did not have current first aid and cpr training.

At arrival time, the opening ratio was observed to exceed the ratio limit and includ infants in care for more than an hour.

In the infant classroom, chemicals were observed to be within reach to children at the sink they use to wash their hands. In the toddler classroom, a radio with a cord plugged in the outlet was observed to be with in reach of the children and is a pull/fall hazard.

The first Aid Kit was missing Scissors.

The Pre-Kinder children do not have age appropriate outdoor play equipment.

Expired Neosporin was in the First Aid Kit. This was corrected when removed from the first Aid Kit.

From January 1st 2024 until May 29th 2024, fire drills have not been conduced monthly.

Water was not made available while the chidlren were outside playing.

Shelter in Place/ Sever Weather Drils or Lock-Down Drills have not been conducted.

The annual fire inspection on file expired on April 8, 2022. The operation did not submit the required fire inspection by the required compliance date. This is a re-cite.

Name and address of the alternate shelter away from the center is not included in the Emergency Preparedness Plan. This was corrected when the director added the name and address of the shelter to the relocation alternate shelter plan.

One of the 3 children's files reviewed, one did not have the Providers Health Statement section of the Admissions Form completed.

Of the 3 children's files reviewed, two did not have this section completely filled out with the required information. Either the name, address and or phone number was not listed.

Cots in two classrooms had numbers, however, operation did not have a number/child assignment map available.

One staff did not have an affidavit and has been employed since October 2021.

Three staff did not have current first aid/CPR training. One did not have any first aid/CPR training. Two staff's certificates expired June 18, 2021.
