One employee who does not have a cleared background check, was observed to be in ratio and caring for children in the PreK classroom.

A two year old child from the toddler classroom, was observed to be alone in the hallway and unsupervised.

A caregiver did not use good judgment when she lifted a two-year-old child by his wrists while redirecting him back into the room when he left the room through an open door.

The operation was out of ratio in the 3 year old classroom with 18 children to one caregiver and a specific age group of 15 children.

Smoke alarm was last tested on May 2024.

Six children's files were evaluated. Three files were pending updated immunization records.

Trash cans in restroom and in classroms did not have a lid. Trash cans contained garbage.

Two infants under the age of 12 months were observed to have blankets while sleeping inside their cribs.

Center did not notify Child Care Regulation a child had an injury that resulted in medical attention.

Caregiver did not use good judgement when she did not follow the operation's procedures, and as a result a child suffered an injury requiring medical treatment.

Parents are not allowed to go beyond the lobby section of the center.

Infant daily reports have not been completed since July 19th, 2025.

A review of the employees who left employment in 2024 data from January 21, 2025, revealed the center had not reported the number of employees who left employment in 2024 by January 15, 2025, as required.

It was found that a fire drill was falsely documented, when the operation has already entered the information for November.

The storgage container in the Toddler playarea did not have hinged lids. This was corrected during the inpection. The staff removed the storage container and placed it in alleyway on the other side of the fence.

The Infant room had five babies with one caregiver. The Infant room was out of Ratio.

Caregiver did not know how many children she was responsibility for.

The Director did not ensure the Infant room ratio was maintained.

Of the 4 children's files reviewed, one did not have the immunizations record available for review. This was corrected bring the inspection.

During the observation in the Infant room, eight of the Infants were crying while one caregiver was documenting and the other holding an Infant.

While in the Infant room a baby bottle with formula was observed in the crib with an Infant.

Both First Aid kits in the operation were missing the Thermometer.

Toys were observed in several of the cribs while the Infants were in the crib. Items were removed when discussed with the owner. This was corrected during the inspection.

Center was operating without Liability Insurance and did not notify the parent in writing.

The teacher in one classroom had walked out of the room leaving the children without supervision. In less than a minute, the caregiver returned. The correction was made during the inspection.
