At the time of inspection 5 children did not have their most current immunization records on file for review.

At the time of inspection 3 children did not have a health statement on file indicating the he or she could be present in a daycare setting.

At the time of inspection 6 of the caregivers/staff present did not have current training over the topic of Poison Prevention for Handling, Storing and Disposing of Hazardous materials and safety trainings.

At the time of inspection the last documented health inspection for the operation was conducted in 7/2024. The operation made an attempt to reach out to the health department to schedule an inspection during the visit.

The Childrens Products Certification Form for recalls was last signed on 5/29/2024. No form was signed in year 2025.

The supporting poles on the outdoors play structures are not marked or labeled to indicate how much depth of loose fill should be present and maintained in the area.

During the inspection, 5 containers of hand sanitizer were left accessible to children in the Sunflower Class. Staff removed these items when prompted by the licensing inspector.

A soiled diaper was located on the floor in the children's bathroom in the Sunflower Room.

During the inspection, an electrical outlet in the Sunflower class was found to be uncovered.

This standard was cited as deficient after cleaning liquids were found in an unlocked cabinet accessible to children in care. The issue was corrected after staff were instructed to remove the cleaning liquids, which were then relocated to a secure area.
