The operation delayed CCR from conducting an inspection when staff took approximately 6 minutes to open the door to the center after knocking, ringing doorbell, and calling the operation's phone number.

The diaper changing mat located in the pre-k room was covered with a type of material that was absorbent and hard to sanitize. Note: This was CORRECTED during the inspection as the cover was removed from the diaper changing mat.

The director did not have current training on Transportation Safety. The training certificate available for review had a date of completion of January 04, 2022.

Some areas of the wooden fence in the outdoor playground had peeling paint. This posed a hazard as the areas where the paint was peeling was within children's reach.

The activity plan posted in the preschool room was not current. The dates on the activity plan was December2024. Note: This was CORRECTED during the inspection as an updated activity plan was posted.

A caregiver caring for a group of children in the outdoor playground did not have the key accessible to the lock on the emergency exit gate. A second caregiver that also went to the outdoor playgorund with chlidren at a different time during the insepction did not take the key to the lock on the gate.

A caregiver was supervising a group of 10 children ages 3mo-3yo. Age difference between youngest and oldest child is over 18 months.

Children's Products Certification Form was last signed / certified on 02/16/2023.

Two caregivers files did not have documentation showing their education. One caregivers file did not have TB results. One caregiver did not have the required 24 hours of Preservice training.

The Playground maintenance checklists were not available for review.

There are no fire drills, sheltering/lock down drills being conducted at the operation.

The front of the slide did not have the required use zone area.

A caregiver in charge of the Preschool group and employee/cook did not have a background check results indicating eligibility to be present at the operation.

Loose fill surfacing (pea gravel) was not observed to be maintained at the proper depth for a play structure in the outdoor play area. The depth of the pea gravel was not marked.

The swings were attached to a composite structure.

The health inspection report was not available for review.

The Emergency Evacuation and Relocation diagram for the rooms in the operation were incomplete.
