Four toddlers were observed in a high chair table without safety straps. This was corrected at inspection when the caregiver put all straps on.

During an investigation it was determined a child was pulled in a form of guidance resulting in them falling to the ground.

During an investigation it was determined a caregiver did not have up to date first aid training.

Caregiver did not intervene when multiple children were playing with bathroom door resulting in injury.

During an investigation it was determined the operation did not report to child care regulation when a child was injured in care requiring medical treatment.

Classroom was observed at time of inspection with 6 children with specified age group under 18 months old. This was one child over ratio. This was corrected at time of inspection when one child was moved into another classroom.

During nap time there was not adequate lighting at time of inspection. This was corrected when the curtains were opened.

During an investigation this standard was found to be deficient. One infant was placed in a bouncer with a blanket in a crib to sleep. One infant was asleep with a boppy pillow and blanket in a crib.

Durning an investigation it was determined infants were sleeping in restrictive devices. One infant was sleeping in a bouncer for over 2 hours with a blanket.

During an investigation one infant was observed in a swing for approximately one hour and thirty minutes.

During the inspection a caregiver was observed on their personal phone.

The staff failed to supervise properly after multiple verbal redirections were ignored by the child to stop climbing on the equipment in such a way that an injury could occur. The child was then injured and required medical treatment.

It was determined that children were outside on the playground unsupervised.

At the time of inspection the operation did not have a current daycare facility inspection.

During the inspection four out of the five infant feeding instructions had not been updated in the past thirty days.

There were several areas along the chain link fence that were pulled up from the bottom and as a result created an entanglement or entrapment hazard to the children.

During my inspection walk through I observed three cans of Febreze and one bottle of cleaning spray low enough for children to have access.

The medication administration log did not include employee who administered the medication.

Ten out of Ten records did not have the required emergency contact.

All classrooms did not have the nap cots labeled or a map for nap cots.

Four infant feeding instructions were not updated in the last thirty days.

Several classrooms did not have a evacuation map posted.

Play equipment had metal springs and bolts exposed.

Six classrooms did not have an activity plan available for review or posted at the time of inspection.

Out of ten personnel files reviewed, four caregivers did not have all of the required annual training certificates available at the time of inspection.

Two safety straps were missing in a infant table that had seats within the table.
