A caregiver was observed screaming and yelling at a child.

It was observed that there are 5 children ages 2 month, 5 month, 5 month, 10 month and 11 month with one caregiver.

Six inches of loose fill surfacing is needed on the playground. It was observed that the mulch is at the lowest level.

Annual fire inspection needed. The last proof of annual fire inspection was dated 7/24/24. The operation was unable to show proof of current fire inspection.

It was observed an uncover trash bin full of trash that accessible to children. This trash bin located in the cafeteria.

The following hazards observed during inspection: An orange electrical cord hanging from the electrical outlet to the wall that accessible to children. Children may pull on it or tripped on it. This cord is locate in the hallway where children walk by regularly. A green electric cord laying on the floor from the outlet to the TV that can pose a tripping hazard. Wall paint tripping from wall in the room next to the four years old class. Dirty carpet in two classroom that accessible to children. Dirty air vent in all classrooms. Uncovered electrical outlet in the four years old class. The electrical outlet also exposed out from the wall. The sink in the restroom that locate in the cafeteria chipped paint, wood exposed and need replace. A section of the ceiling in the three year old room cracked open. The sink counter in the three year old room need replace. Paint in the hall way chipped in a large areas. Several paint chipped from the ceiling in the three years old room. Door to the "Restricted Area" was not stable, the door is not close properly. Nails were exposed in several places from the outside climbing structures. Severl wood panels are broken that pose a risk to children.

Caregiver did not use good judgment when placing empty milk bottles in the sink that designated for handwashing in the infant room.

This standard was evaluated by Child Care Investigator and has been found to be in violation with the Minimum Standard Rules for Licensed Child Care Centers. Caregivers used force when spanking the children, which resulted in multiple bruising on their buttocks. This is considered as physical abuse due to significant bruising.

Based on Child Care Investigator, it was determined that a caregiver was spanking two children that cause bruises on their body.

Four out of ten children's record reviewed, four of them were missing the health statement.

It was observed that nails are protruding from climbing structure that may cause injury to children.

It was observed a loose fitting sheets in one of the cribs in the infant room.

It was observed that caregiver let children under two year old watching a music cartoon clip from an ipad. This is corrected at inspection.

It was observed that caregiver did not wash a two years old child after changing his diaper.

Three caregivers did not have 24 hours of annual training hours.

Six inches of loose surfacing need around the climbing struture.

Two caregivers did not have proof of CPR and First Aid certified.

An Affidavit for employment is needed for one caregiver.

One out of ten chlidren's record review was missing information regarding emergency medical authorization.

Proof of education needed for two caregivers.

One out of ten children's record reviewed, one was missing copy of immunization record.

There were six infants ( 6m, 6m, 6m, 11m and 16m) in the infant room with one caregiver. They over one child over the child/staff ratio.

Current CPR and First Aid needed for a caregiver who is in charge of a group of children.

At least 6 inches of loose-fill surfacing material is needed for the big playground. It was observed that weeds and grass are growing from the areas surrounding the slides.

It was observed that the metal fence in the toddlers playground was broken. The top parts were loose and not stable.

Two of the ten children's records reviewed did not have a health statement on file.

The operation does not have a current fire inspection. The previous fire inspection was conducted under the leadership of the previous owner.

Buse LFH-4081 does not have a secured fire extinguisher.

The playground structures has a piece of equipment with protruding screws.

There was not an adequate amount of surfacing on outside play area.
