It was found during a self-reported investigation that a child was able to open an exterior door unnoticed and go outside unattended for approximately 1 to 3 minutes.

Out of 10 children records reviewed, 7 did not have the required Health Statement.

Out of 10 children files reviewed: 2 children did not have immunization records, 1 child was missing proof of 1 of 3 Hep B vaccines, and 1 child was missing proof of 1 of 4 Dtap and 1 of 4 PCV vaccines.

While a class was transitioning from the playground to the classroom, a child left the line to use the restroom. The teacher did not notice until the child was brought back to her 4 minutes later.

It was found during the inspection a caregiver did not have an active background check at the operation.

It was found during the investigation the operation failed to report a situation that placed a child at risk within the required 48 hours.

It was found during the investigation a caregiver pulled a child's hair as a form of punishment.

It was found during the investigation a caregiver left a child in a classroom unattended.

It was found during the inspection the operation had sippy cups/water bottles that were not labeled with the child's name. This was corrected at the inspection when the caregiver labeled them.

It was found during the inspection the pre-k playground had a climbing structure that was breaking causing sharp pieces of metal to stick out.

During an investigation, there was sufficient evidence to support that a child was left unsupervised in the classroom for approximately less than 30 seconds. This was corrected when the staff notified the leadership team and corrective measures were taken. The operation provided an all staff training on supervision the same day. The operation has also implemented paper name to face sheets to ensure staff are aware of their group at all times. Management is also conducting regular checks in the classrooms at least one a day to ensure staff are keeping track of name to face sheets.
