A caregiver did not follow name to face procedures when transitioning from the outside playground to the classroom. As a result, a child was left outside on the playground unsupervised for approximately 4 to 5 minutes.

Two caregivers did not follow procedures of name to face for all children when transitioning from the classroom to the outside playground. As a result, a child was left inside the classroom unsupervised for approximately 11 minutes.

An employee did not use good judgment when management was not immediately notified of an incident that occurred.

Based on information gathered during the investigation and a review of video footage, a caregiver failed to provide comfort to a toddler who was visibly upset during drop‑off times.

During morning dropout toddler walked out of the Two A classroom due to the door being left open by a parent dropping off.

Based on information gathered through the course of the investigation, it is determined that children at the operation have been inappropriately grabbed by caregivers.

Based on information gathered during the investigation, it is determined that caregivers have been using their personal devices while caring for children.

Based on information gathered during the inspection, children in care went more than 3 hours without a meal or snack.

Based on information gathered during the course of the investigation, a caregiver did not demonstrate competency by ensuring the Pre-B schedule was followed and children in care were fed before being instructed to lie down for naptime.

Based on video footage reviewed and information gathered from interviews, a caregiver positioned a toddler in front of a chair when they were not listening. The caregiver then sat down in the chair with their legs enclosing the child beneath them.

Based on information gathered during the investigation, a caregiver has inappropriately grabbed children in the toddler classroom.

Based on information gathered during the investigation, it is determined that a toddler caregiver hit a child's hand while in care, and has hit children's hands in the past.

Based on information gathered during the course of the investigation, it is determined that caregivers in the operation?s 1B, 2A, 2C, and 3B classrooms have been witnessed inappropriately grabbing and pulling children. One caregiver was observed pulling a child up off the floor by the collar of their shirt.

Based on information gathered during the course of the investigation, it is determined that caregivers in 1B, 2A, and 3B have been observed yelling in the children?s faces and using aggressive language. It is determined that the caregiver in 3B has used other children to taunt and humiliate children who wet themselves, misbehave, etc.

Based on information gathered during the investigation, the operation has two subjects with active background checks who have not been employed there for over 7 days.

Based on information gathered during the course of the investigation, it is determined that approximately since May of 2024, the director and assistant director have been knowledgeable of multiple ongoing concerns of minimum standards violations at the operation that places children at risk.

Based on information gathered, it is determined that an infant/toddler caregiver has frequently not promptly changed children?s diapers resulting in diaper rashes and dried poop being found on them by other caregivers.

Based on information gathered, no incident report was created for an incident involving a child being placed out the back door of the classroom and being left outside with no direct supervision. The incident reported was not created as of 6/25/2025.

Based on information gathered during the course of the investigation, it is determined that the operation has allowed children with a fever to remain in care. When children are sent home with a fever, they are not required to remain out of care until 24 hours symptom-free.

Based on information gathered during the course of the investigation, it is determined that caregivers in the operation?s 1B, 2A, 2C, and 3B classrooms have been witnessed inappropriately grabbing and pulling children. One caregiver was observed pulling a child up off the floor by the collar of their shirt.

Based on information gathered, no incident report was created for an incident involving a child being placed out the back door of the classroom and being left outside with no direct supervision. The incident reported was not created as of 6/25/2025.
