Based on the evidence obtained during the investigation, a caregiver was not supervising appropriately. A child left the classroom without the caregiver realizing until discovered by another caregiver.

Based on the evidence obtained during the investigation, a caregiver was not disciplining appropriately when grabbing a child by the arm.

In 2 of the 4 files reviewed staff's CPR/First Aid certificate was expired. As well as several other staff. There were 2 staff with current First Aid and CPR certifications.

Based on information obtained a child was grabbed by a caregiver as discipline, child was not injured.

Based on the information obtained during the investigation, a group of children were left alone in a classroom for approximately 1 minute and 42 seconds.

A child was left alone briefly without supervision.

Based on information obtained a parent was not notified of an incident that placed their child at risk, immediately, after child was safe.

Based on information obtained a child was at risk due to a lack of supervision and the incident was not reported to licensing within the required time frame.

Based on the information obtained a child was unattended in a classroom for at least 10 minutes without supervision.

The annual fire inspection was due 2-9-2023.

A caregiver was observed changing a child and walking away from the changing table, removing their hand from the child on several occasions.

It was observed that several children were changed and the changing surface was not sanitizing.

The operation allowed a person to be present and with children at the operation prior to receiving a background check determination of "Eligible, with conditions, or provisional.

There is evidence to support staff not washing their hands or the children's hands according to the procedures.

There is evidence to support that there was one staff member with 20 children that had a median age of 3 years old. The ratio for 3 year old's is 15 to one.
