One of the booster seats in a transportation vehicle has expired.

Based on the information gained during the investigation, during the summer the operation played the following films which contain violence: Spider-Man: Into the Spider-Verse, Kung-Fu Panda 4, and Sonic The Hedgehog 3.

There are several broken slats in the playground's wooden fence. A television was observed on a shelf in the infant room. The television was not in use but was not anchored to the wall or the shelf.

One of the driver's First Aid/CPR certification expired in 09/2024.

Observed a climbing dome in the outdoor playground without any surfacing at the time of inspection.

The wood flooring in children's outdoor playground was not maintained. One 2 X4 board had a hole size of a pen in diameter accessible to the children at the time of inspection. A little tykes bike was also observed with a cracked wheel accessible to the children.

Based on the information gained during the investigation, one caregiver did not ensure all children were accounted for during transportation leaving a child unsupervised in the vehicle with the windows up which could have resulted in death.

Based on investigation by DFPS there is evidence that one caregiver was found to have neglectfully supervised a child in care.

The operation is not complying with all of the conditions imposed by the corrective action plan. Conditions 3, 4, and 5 have not been met within the required time given to the operation.

Observed one caregiver supervising 5 infants ages 7 months, 7 months, 11 months, 13 months, and 13 months. Caregiver was over the ratio by one. Specified age group was 0-11 at the time of inspection.

A review of the Employees Who Left Employment in 2023 data from January 22, 2024, revealed the center had not reported the number of employees who left employment in 2023 by January 15, 2023, as required.

A 3 year old was observed in a restrictive device (high chair) Caregiver advised that child is active and does not sit and child is interchanged with the sitting in the table and high chair.

Observed one ceiling tile in the afterschool classroom torn and three other tiles with (black marks of water stains) at the time of inspection.
