Of the six children's files reviewed, one did not have updated immunizations.

A caregiver did not use positive discipline when she scared children into behaving properly by telling them that a monster lives in the daycare that will eat them.

Seven of the eight files I reviewed did not have updated immunization records.

None of the eight children files I reviewed had emergency doctor or hospital information.

Six of the eight children's files I looked at were missing their statement of good health from their family doctor.

One teacher did not have proof of her high school dipoloma.

A new acting director was present without a background check. She left the operation during the inspection.

There were 3 caregivers present at the operation who did not have background checks. They left the operation.

Three caregivers had online training for CPR training.

The director left her job suddenly and did not appoint someone to be in charge after she was gone for the day.

Two classes combined for naptime and stayed that way the remainder of the day on 6/12/2024. Five children ages 3 to 5 years old were mixed with 9 children ages 14 months to 21 months old.

All four caregivers working on May 17th report that a child did not have their temperature taken even after being asked by the teachers and parent. By the time the parent picked up, the child's temperature was 104.

There were four infants without signed and dated infant feeding schedules.

There was not an allergy action plan signed by a doctor for a child who has an Epi-Pen.

The director did not her training record available for review during inspection.

There was no a carbon monoxide detector that was installed at the time of the inspection.

The fire extinguishers were last serviced in October of 2022.
