An accidental injury occurred on 4/10 and the operation was aware the same day that child received medical care but did not report the situation to CCR until 4/15.

The child wasn't using the play equipment appropriately by sitting on top of the monkey bars. The child sustained the injury when going through one of the openings in the bars hitting his mouth. Staff saw the child sitting on top of the monkey bars and didn't intervene to ensure the child's safety.

A child with special needs was observed playing on a Nintendo Switch during today's inspection.

The program doesn't have current documentation of a completed playground checklist.

A boy was observed trying to go into the girls restroom. Three girls were unsupervised playing in the restroom.

An older child was observed standing in the hallway watching a younger child walk down to the gym instead of the caregiver.

Doctor's signatur e is need for a child's food allergry and anaphylaxis emergency care plan. 1 child needs and food allergy and anaphylaxis emergency care plan signed by the doctor and parent for listed food allergy that requires and epi-pen and be priovide the required medications to the program.

Children in the cafeteria during snack were allowed to walk out of the cafeteria unsupervised to walk down the hall to use the restroom.
