The outdoorplay area fence was observed broken with nails sticking out. This was corrected at inspection when the operations contractor nailed the wood back into place and removed the nails.

Based on the evidence gathered during the investigation, there is sufficient evidence to conclude that the infant classrooms sink was broken and not equipped with running water.

There was an outlet without a cover in the toddler room. This was corrected during the inspection when a cover was put in the outlet.

Out of four staff files reviewed, all of them were missing a pre-employment affidavit, one was missing their diploma, and two were missing statements acknowledging that they received the center and staff policies and attended orientation with an overview of abuse and neglect procedures.

The handwashing sink in one of the infant rooms was out of order, and the portable handwashing sink in another infant room had been moved out of the room.

Out of four children's files reviewed, one was missing information for an emergency medical facility, one was missing the phone number for their school, and one was missing a health statement.

The operation did not have a menu posted for the week. This was corrected during the inspection when the weekly menu was created and posted for parents to view.

The operation added an outdoor space without notifying Child Care Regulation. This was corrected during the inspection when the new outdoor space was inspected and measured.

There were two outlets in the eating area that did not have outlet covers. This was corrected during the inspection when outlet covers were placed in the outlets.

During the inspection, one of the teachers stepped out of the room to throw away garbage when there were 12 children in the room with a specified age of two years old, leaving the room out of ratio. This was corrected during the inspection when the caregiver returned to the room.

There was a caregiver present in the classroom who did not have a cleared background check yet. This was corrected during the inspection when the caregiver was sent home.

Several cots were not labeled with the children's names.

During naptime, two infants ages 15 and 16 months old were combined with children up to nine years old. This was corrected during the inspection when the two infants were moved to the infant classroom.

The parent handbook did not have contact information for licensing, procedures for providing services to special needs children, information on employee vaccinations, and criteria for when extreme weather will prohibit outdoor play.
