The menu posted did not reflect what the children ate for lunch today. The operation has a rotating menu and the substitution was not documented.

A caregiver did not have verification of pre-service/oriientation on file for review.

A caregiver only had 21 out of the 24 hour annual training required. A caregiver had 10 of the 24 required annual training. A 3rd caregiver only had 3 of the 24 required hours on file for review.

A classroom was observed to have water damage on a wall under the air conditioner, trash on the floor in the bathroom and near a trashcan by the sink, crumbs on the table and floor, their were 2 light fixtures hanging from the ceiling and a sofa was observed to have fraying.

There were 3 children that did not have a health statement on file for review.

A caregiver's file did not have a notarized affidavit on file for review.

The operational policy was reviewed and the following sections were not observed in the policy: infant safe sleep, policy changes and insect repellent/sunscreen.

The director had only completed 10 out of 30 required hours of annual training.

Soiled diapers were observed in a trashcan with no lid and the diapers were not observed in individual closed bags.

There were rodent droppings observed under the sink in the infant classroom.

A caregiver was observed not to have washed an infant's hands after changing the infant's diaper.

A caregiver stated she had 14 children in care and it was determined she had 17 children in care.

The child safety alarm was observed to not be working properly when tested.

There were 2 caregivers that were unaware of the ages of children in their group. The caregivers were unaware where to the find the sheet that contained the information.

The operation did not have a plan to ensure the supervision of children, if the transportation was involved in an accident. After the accident, the driver left the children unattended in the vehicle, to check on the other vehicle.

It was determined the operation did not notify Child Care Regulation of the incident that placed children at risk.

During the inspection on 5/03/24, the Director did not have documentation of transportation training for herself.

It was determined the driver of the transportation vehicle did not utilize good judgement when he attempted to pass another vehicle and did not ensure there was enough clearance. During attempting to pass the vehicle, the driver hit the other transportation vehicle, that also had children in the vehicle.

A caregiver stated she had 27 children, during naptime, and it was determined she had 28 children in her group.

Two children were observed utilizing/viewing a tablet during naptime. The tablet was removed from the children.

Two classrooms did not know the ages of the children they were caring for. One classroom stated that the youngest was 14 months and the oldest was 4 years old. The correct ages of the children were youngest being 20 months and oldest being 3 years old. The second classroom was askeed about the ages of the children and the caregiver stated that they did not know how old the children were that were in the caregivers classroom. I explained to both classrooms that they must know how many children and the ages of the children in their care.

An infant was found asleep in a crib with a bottle The bottle was removed from the crib during inspection and I spoke to the caregiver about not allowing infants to sleep with bottles or cups.

There were two infant bottles that were not labeled with the infants first name and last initial.

A caregiver had unsupervised access to the children in care without a cleared background check. This was corrected when the caregiver was asked to leave the operation. An administrative penalty will be recommended as a result of this citation, per HRC ?42.078.

The operation was observed to have not completed a building grounds checklist since the beginning of December 2023.

There was a 70 inch television in the school age room that was not anchored.

I observed a toilet that needed a new toilet seat, the school age classroom was observed to have food and crumbs on the floor, there was a used paper towel near the sink and paper pieces on the floor, the restrooms had a ring around the toilet and one toilet was observed to have feces that had not been cleaned. There was toilet paper observed on the floor of an indoor restroom and in both of the restrooms in the outdoor classroom.

There were 2 infants that did not have updated infant feeding instructions on file.

There was 1 child in care, that has a food allergy and there was not a food allergy plan on file for review.

It was determined a caregiver was heard multiple times yelling at children in care when they were not following instructions.

During an investigation, dead bugs were observed on the window seals.

It was determined a caregiver restrained a child with their arms and legs to prevent the child from moving after being disciplined.

Information from a DFPS investigation supports that a threat of corporal punishment was made and children have been spanked.

Observed the infant bottles being washed in the sink that is also used for washing hands after diaper changing. This was corrected when I spoke to the director and caregiver about cross contamination.

Observed 8 children files and 5 children did not have health statements on file.

During an investigation it was found a caregiver was left to supervise a group of 15-18 two-year-old children alone, placing the caregiver out of ratio by 4-7.

During the DFPS investigation it was found caregivers spanked children in care.

A classroom did not have a caregiver in the classroom when a walkthrough was completed. This was corrected when a caregiver came in to the classroom.

A 5 month old infant was sleeping in a restrictive device. This was corrected when the infant was woken up.

A person was observed at the operation with access to children in care, prior to having a background check submitted.

There were 3 caregivers that did not have current CPR/First Aid training on file.

There were 2 infants that did not have updated infant feeding instructions on file.

A diaper changing pad in a classroom was observed to have several tears, making the changing pad non absorbent.

The operation had not completed a fire drill since February. A fire drill was completed during the inspection, bringing the deficiency into compliance.
