An 8 month old infant did not have current feeding instructions on file.

4 children were missing immunizations at the time of inspection.

One employee did not have current transportation training on file. It was corrected during inspection.

The director file was not at the operation. Copies or any other documents were not provided upon request.

The smoke detectors were not installed.

A caregiver did not follow the required procedures while supervising a group of children, as a result a child was left unsupervised in a room.

The preschool class arranged during naptime did not allowed caregivers access to a walkway without having to walk on or over the cots or mats of other children

The preschool room has a wall that dived the room in two sections, only one caregiver was supervising both sections during naptime, this arrangement did not allowed the caregiver adequately supervise all children in the group.

Based on the investigation findings, this standard was reviewed and found to be in non-compliance. There have been two deficiencies in ratio within the past two months, it is the director?s responsibility to ensure the center is operating in compliance of the Minimum Standards for Child-Care Centers.

On 03/22/2022 the Infant B classroom with a specified age group (SAG) of 12-17 months old was over ratio by five infants, the child to caregiver ratio was 10:1. On 03/23/2022 and 03/24/2022 the Infant B classroom with a SAG of 12-17 months was over ratio by three children, the child to caregiver was 8:1. On 03/22/2022 the Infant A and EPS classrooms were over ratio. The Infant A classroom with a SAG of 12-17 months had a child to caregiver ratio of 12:1. The EPS classroom with a SAG of 2 years old was over ratio by seven, they had a child to caregiver ratio of 18:1.

The infant b classrom with a specified age group of 12-17 months was oberved to have 6 children in the classroom. one child was picked up during the inspection.

Based on the investigation findings, this standard was reviewed and found to be in non-compliance. The caregiver inappropriately disciplined the child on two occasions by shaking the child back and forth. This incident occurred while in the STEM Lab, the caregiver is no longer employed at the operation.

Three of the ten children's files do not have preschool health statements on file.

Based on the investigation findings, this standard was reviewed and found to be in non-compliance. The operation had not reported two COVID-19 cases within the appropriate time frames. The operation was notified on 9/12 and 09/13 but did not report to CCR until 09/18.
