All cleaning spray bottled were found labeled and stored out of the reach of children. First aid kit, suntan lotion and bug sprays were also stored in cabinets. The spray disenfectant was not removed from the unlocked teacher drawer in the prek room.

Two classrooms had spray bottles labeled bathroom sanitizer on the classroom sinks accessible to children. There was also a bucket of various sun tan lotion sprays on the sink counter in one room and in another room the first aid kit was sitting on the sink counter with first aid supplies inside. One teacher desk had an unlocked drawer with disinfectant spray inside.

A caregiver with a pending background check was observed in direct care of four children. NOTE: The caregiver was sent home and instructed they could return to work once the operation receives an eligibility letter from CBCU.

A 14 mos old child was observed standing up in a bucket seat that rests in a horseshoe table. The safety strap was missing in the seat.

An infant caregiver was observed listening to and watching a video on their cell phone while feeding an infant.

A caregiver was observed placing an 8 month old infant on their side in their crib to nap and had a blanket between the front of the infant's body and the back of the crib while patting the infant to sleep.

A toilet in the preschool area was not functioning and had been covered with a plastic bag. The fence gate off the back playground leading to the field would not latch all the way, had fire ants in the latch, and the fence was leaning. The red fire truck on the back playground had a warped board causing a possible trip or entrapment hazard. The fence gate between the two playgrounds was broken on the bottom of the gate exposing sharp metal.

A caregiver was observed placing an 8 mos old infant on their side to nap.

An employee used poor judgment by taking their child into the kitchen while working in the kitchen.

An infant was observed swaddled while in their crib and the operation did not have a sleep exception form on file.

A caregiver used poor judgment when they brought an infant outside of the building with them to retrieve school-age children from the public school bus.

The operation did not have Liability coverage at the time of the inspection.

The operation's Emergency Preparedness Plan was missing information regarding how staff will continue to care for children at the relocation site until parents can pick up, how parents will be reunited with their children, how the operation will communicate with parents and local authorities

A toilet paper holder in the back bathroom needs to be re-secured to the wall. A metal brace in the 18-24 mos room restroom used to hold cleaning supplies was not secured to the wall and falling forward.
