The annual fire inspection report was not available for review. The current report expired on 06/11/2026.

During the inspection it was observed the nap mats were not labeled with the children's names and nap charts were not available/being used in three of the four classrooms being utilized at nap time.

The operational policies did not include procedures for supporting inclusive services to children with special care needs. The CCR section also needs updated and a section for preventing to abuse neglect needs added.

2 out of 5 employees evaluated were short the required 24 hours of annual training. One was missing 4 hours and one was missing 7 hours.

Based on the findings of a DFPS investigation, a caregiver exhibited poor judgment when they acknowledged not contacting the management team when they should have after becoming frustrated dealing with a child?s behavior in the classroom.

Based on the findings of a DFPS Abuse/Neglect investigation there was a finding of reason to believe for physical abuse regarding a child who was inappropriately disciplined.

Based on the findings of a DFPS Abuse/Neglect investigation, a caregiver was aggressive with a child when disciplining them and forcefully moved them, put their head down forcefully on the table, tapped the child, and grabbed the child by the arm/shoulder. These are prohibited forms of discipline/guidance/punishment.

Based on the information obtained during the DFPS investigation there was sufficient evidence to show that a can of Febreze was accessible to children as the Febreze was on a low window ledge and children were in the classroom.

Mulch was observed not to be maintained at the proper depth underneath the big playground structure area. There was at least 2 inches observed underneath the pole that was marked for the depth.

I observed a broken toilet in the facility that needs fixed. I observed gnats flying around in a few of the sink/toilet areas.

One classroom was out of ratio during the inspection. This was corrected during the inspection when additional staff members arrived. Information gathered in the investigation substantiated that the operation has exceeded child/caregiver ratios in different classrooms at times in the past couple of months.
