The center did not have the required Licensing Parent Notification posting.

The center's health inspection is past due. The center's last health inspection was condcuted April 11th 2025.

This standard was tasked in relation to a DFPS investigation. A caregiver did not use positive redirection and guidance when separating two children, resulting in redness and bruising to one child's arms.

A child was accidentally left alone in a classroom.

A ball cart fell onto a child requiring the child to need medical attention.

The fire inspection on file has expired.

Based on the preponderance of information obtained during the investigation a caregiver used poor judgment while handling children in a classroom. Note: The child was not injured. The caregiver was immediately suspended until the investigation is completed. All other caregivers were retrained on positive methods of guidance.

A Director was informed of concerns relative to a caregiver's actions that resulted into continuing to use poor judgment when dealing with toddler age group. The Director did not ensure the caregiver receive additional training on how to properly handle or care for the toddler age group.

During nap time a cot was placed in front of the door. This was corrected when the cot was moved. A picture was taken. It was in toddler one classroom.

In several restrooms there was trash. Pictures were taken. This was corrected during the inspection when the trash was picked up.

Based on the information gathered from the DFPS investigation supervision was not met when two children left the playground and wandered off into the bushes without their teachers' knowledge.

In the toddler classroom one teacher was asked how many children she had. She stated 7. The other teacher present stated 8. It was verified by CCR their were 8 children present. 08/31/22 Follow-up inspection - A teacher in the preschool four was asked how many children she had. First she stated 14 then stated 15. It was verified by CCR there were 15 children.

Parents of the child involved in the incident were not notified of the allegation of abuse.

Based on the information gathered a child was abused when a teacher grabbed a child by the upper arm leaving two red linear marks.

The current fire inspection on file expired on 02/25/21.
