The operations classrooms and active indoor play area were observed to have broken walls, holes, and chipped paint that is accessible to children of all ages.

One child care been enrolled at the operation for over two months without a completed food allergy plan on file.

During the inspection, the operations' outdoor play equipment was observed to have chipped paint and sharp cracks, accessible to children.

The operation does not have required accommodations information on file, for one child in care.

Operation has not fixed the coating on the play structures. Contractor needed to move the date of completion.

During the walkthrough, two playgrounds were observed with play stucture platform coatings starting to break off. The paint on the play structures is tearing off in multiple places.

During the walk through on the school age playground, the mulch did not measure the required 6 inches.

A caregiver did not use good judgement when raising their voice and using inappropriate language at a parent while a child was present.

One childs food allergy action plan was observed and was missing the following; The action plan signed by the parent and the ePipen on file.

Ten children files were missing the following under the special care statements; (A) Any limitations or restrictions on the child's activities; (B) Special care the child requires, including: (i) Any reasonable accommodations or modifications; (ii) Any adaptive equipment provided for the child, including instructions for how to use the equipment.

On the school age playground, under the parallel bars area, there was not the required 6 inches of mulch.

The playground fences were observed and the bottom of the fences were coming up and poking out in different spots.

During the inspection, one caregiver allowed a child to walk into another classroom and did not know how many children were in care. During the inspection, multiple children were observed going back and forth to lockers without caregiver supervision.

Based on the inspection conducted, the playground does not have the required 6 inches of mulch.

Ten childrens files were observed at the inspection and missing the following; -Two childrens files is missing a special care statement, -One childs file was missing health care information.

Three childrens files do not have updated immunization records.

There is a tear in the back of a seat on one of the centers vehicles.

On the outside facing the school age playground, there is a window security bar hanging off the window and pieces of the gutter are on the ground. In the indoor play space there are broken plastic windows, a broken border piece, and walls with chipped paint.

On all three playgrounds, the play stucures platform coatings have broken off or torn off. On the school age playground equipment, the walking bridge has broken pieces. On the toddler playground there was a broken toy car.

The children's nap mats in the classrooms were not labeled to each individual child at the center.
