Observed was cleaning supplies accesible to children. Gnats were observed to be inside and flying around the trash inside of the kitchen area.

The left side of the backyard fence has two loose boards.

Observed unkept carpets, floors not clear for the children to walk, and exposed drywall all accessible to the children in care. Ceiling in afterschool classroom is leaking.

Observed the hallway wall with a hole accessible to the children at the time of inspection.

Observed the pre-kinder classroom being used as a storage room and not accessible to the children in care.

Observed two little tykes home used by children broken and accessible to the children at the time of inspection.

Observed the diaper changing equipment being used as a shelf to place other items other than changing children's diapers. Observed a child's diaper being changed on the floor at the time of inspection.

There was no record of sign in/out logsheets for the past three months.

One employee record was not available at time of inspection.

The provider did not have emergency medical authorization for 1 child who requires transportation. There were also 2 children on file who required transportation that did not have transportation authorization. Two other children who accompany the provider to pick up the school-age children also did not have the required authorization for transportation.

One out of one employee records did not have child maltreatment, SIDS, shaken baby, early brain development, administering medication, and food allergy training on file. There were 22 out of 30 hours for the annual 2020-2021 year.

The outdoor play area needs to be maintained and cleaned. There is a missing board on the outdoor fence in the backyard as well as toys that need to be cleaned. The back room was observed with dirt and debris. The indoor kitchen area had bins that needed to be removed. The living room area had chairs and other items that were stacked. One child was observed knocking the chairs over. The hall way had a "roller coaster" toy blocking the walk way. Toys at the operation as well as the first aid kit were observed with dust and dirt. A first aid bin also had a dead fly on the bottom of the bin. The transportation vehicle had trash, leaves, and pencil shavings in the cup holders and under the seats.

One out of 5 children's records on file did not have enrollment paperwork on file including no name or date of birth. One out of five children's records did not have enrollment paperwork on file however other paperwork did have a name and date of birth.
