Two caregivers were providing care for 36 children ages 3-5yo, SAG 4yo.

Rug in classroom 3 has multiple dirt stains and diaper changing mat is torn. Cleaning products in toddler - prek age classrooms are stored on top of the counter and accessible to children. There are three moisture absorbers by hall and accessible to children. There is a large stain on the wall in room 7, by the hand drier. Note: cleaning products and moisture absorbers were removed during inspection.

One of the ten childrees files reviewed, did not have the Address of the Emergecy Contact listed.-

Staff files were not made available for review.

The Health Inspection has expired. Payment was made for the inspection however a date has not been set for the inspection.

The First Aid Kit was missing Gloves. When this deficiency was brought to the attention of the person in charge, she immediately placed a pair of gloves in the First Aid kit.

In one of the classrooms, garbage was made accessible to the children by not having a lid on the Trash Can. This was corrected when the director placed a lid on the trash can.

Of the five children's files reviewed, one did not have the Operational policies provided.

There are two areas of the Unitary surfacing on the playground that need to be repaired. Photos were taking for documentation purposes.

Four of the five children's files reviewed did not have the Health Care Provider Statement.

One of the Five Children's files reviewed did not have the Immunization Record.

Of the five childrens files reviewed, one did not have the Name, Address an Phone Number for the Emergency Contact. This was corrected when the Director looked up the child's information on PRO CARE and add the infromation to the Enrollment Form.

In the toddler room there was a peanut allergy with no allergy list posted.

Based on the information gathered and preponderance of evidence, a child's EpiPen was expired as of March 2021, and documented on the Medication Authorization Form dated 2/18/20 - 3/18/21.

Based on the information gathered, the child's food allergy was not posted in his newly assigned classroom.

Based on the information gathered, a caregiver did not share the child's food allergy information with the new assigned caregiver.

One caregiver had two classrooms for a total of 23 children ages 4 and 5 years. The second caregiver arrived within a few minutes to separate the 2 classrooms. The correction was made during the inspection.
