An infant was sleeping in a play yard that did not have a firm, flat mattress that snugly fits the sides of the play yard.

A 9 month old was sleeping a play yard that had stuffed toys in it as well as a blanket hanging on the side of the crib. This was corrected when the caregiver removed the soft toys and the blanket.

The fence in the backyard is broken, exposing sharp wooden edges that poses a safety hazard to children.
