An infant was observed sleeping with a bottle during the inspection.

During the inspection, the safety straps were left unfastened for 2 infants sitting in high chairs.

Based on the information obtained, a caregiver did not know the correct number of children they were responsible for when transitioning back into their classroom.

Based on the information obtained, a child was left unsupervised on the playground by their caregiver.

Two medications no longer used were stored in the medication box. Note: this was corrected at inspection when the director placed the medication in the front to be given back to the parent.

1 food allergy emergency plan is missing the parent signature.

The school age building is missing the emergency evacuation and relocation diagrams in 3 classrooms. Note: this was corrected at inspection when the director placed the diagrams in each classroom.

The center's last fire inspection was 1/25/24.

There is a caregiver present in the Infant 2 classroom without a renewed background.

It is alleged that a child was left in a highchair for an excessive amount of time.

A child in care was injured and required medical attention. The operation did not report the injury to Childcare Regulation.

Proof of education documentation for two employees were not available for reviewed during the inspection.

The shot record for a 5 month old infant was not available for review during the inspection.

The infant feeding instruction for one 6 month old infant was observed has not been certified since April 2024.

During the inspection, I walked into a classroom and the caregiver was on a personal device and the caregivers back was turned.

During the inspection the operation had not conducted the August, September or the October monthly test the Carbon Monoxide Alarm Detector.

During the inspeciton the operation did not have documenation of the gas leak inspection report being conducted.

During the inspection the operation had not conducted the August, September or October monthly test of the smoke detector.

During the inspection the operation had not conducted the August, September or the October monthly test of the Fire Extinguishers.

The operation did not have the most recent sanitation inspection report available to review during the time of the inspection.

A primary driver at the operation who transports children did not have updated transportation training available during the inspection.

The operation did not have an annual updated Children's Product Certificate available during the inspection. Note: This was corrected.

Six out of ten infants did not have updated infant feeding schedule available to review during the inspection.

Based on the information obtained during the investigation, the caregiver used prohibited punishment towards a child during naptime.

It was observed that there was a total of 18 children with one caregiver during naptime. The children were not combined in a central sleeping area. The operation was out of ratio as one caregiver can only care for maximum of 11 children. NOTE: This was corrected, an additional caregiver was added to the classroom during the inspection.

The operation failed to submit the background check renewal for one employee that were present and in contact of children in care.

The background check status for one caregiver in the Infants classroom was noted "Inactive" since 11/29/2021. The caregiver was observed caring for children during the inspection. NOTE: The caregiver left the operation during the inspection
