The playground loose fill is not maintained prorperly.

Upon arrival to the operation I observed one caregiver supervising 12 children with the youngest children ages 13 months, and two 16 months children. The oldest child age is 11. The children were observed in the group ratio in the morning at 8 am.

An infant caregiver was supervising 6 children ages 4, 8, 13,17,15, and 18 months old.

The operation staff/children are not exiting the building during fire drills within the required 3 minutes. Most of the documented drill (January - April 2022) exit times have been in excess of 10 minutes.

The operation did not have several of the required items inside the van used to transport children. The following items were missing: -Emergency medical transport and treatment authorization forms for each child - emergency telephone numbers for each child being transported

2 out of 10 children did not have a statement from a health care professional.

3 out of 10 children did not have addresses on file for their emergency contacts.

The classroom did not have posted a an assigned mat/cot chart and the mats were not labeled.

Several of the personnel files evaluated were missing the following: 2 out of 10 records was missing a date of hire 1 out of 10 records did not have an employment affidavit 2 out of 10 records did not have proof of education qualification

The operation had a copy of the signed food allergy emergency plan but did not have the prescribed medication (Epi Pen) from the physician for the child. Instead, the operation had a TRAINER epi pen in the child's medication bag along with the food allergy emergency form.

Four infants did not have up to date feeding instructions signed by a parent.

1 out of 10 caregivers who have access to children does not have pediatric CPR training.

1 out of 10 caregivers who have access to children does not have pediatric first aid training.

A caregiver caring for infants admitted to leaving a bottle in the crib with the infant.

Operation staff reported the child was not feeling well and slept for majority of the day. Staff did not notify parent or share with the parent in writing or electronically that the child was not feeling well or slept all day. There is no documentation on the printed Kindercare App indicating the parent had been notified.
