Six infants was observed in the infant classroom with two caregivers. Note: One caregiver reported just arriving in the classroom prior to Licensing entering the building.

A child was observed drinking a bottle while laying in a bouncer seat. Note: This was corrected when the caregiver removed the child from the bouncer seat.

Two infants was observed not strapped in while sitting in a bouncer seat. Note: This was corrected when the caregiver removed the children from the bouncer seat.

A child was observed sleeping in a restrictive device. Note: The caregiver removed the child once it was called to her attention.

During the follow up, the television was on in the Toddler classroom when it was not in use. Note: The television was turned off during the inspection.

As of today, the operation still has not had the annual fire inspection completed since January 29th, 2025.

A caregiver did not know the correct number of children they were responsible for during the inspection.

A caregiver placed a child in a highchair when the child did not want to lay down and take a nap The child was able to climb out of the highchair.

A caregiver grabbed a child by the arms, forcing the child down and when placing the child on their nap mat; the child hit the back of their on the nap mat.

A caregiver was observed forcing a child to sleep by making a child laydown on their nap mat.

A caregiver placed a child in a highchair and put the child in the hallway, closed the room door, and walked away leaving the child unsupervised.

A caregiver served children food while they were in their cribs and laying down on their nap mats.

A caregiver placed an infant in a baby bouncer and did not fasten the safety straps. The infant almost flipped out of the baby bouncer.

A caregiver was observed on video hitting a child on the arm.

Based on the information obtained, children were watching television during naptime.

Caregivers were using their personal cell phones to talk and text while caring for children.

Children did not have their hands washed before eating.

Observed children under 2 engaged in screen time during the inspection. Note- The operation turned the television off.

The annual fire inspection has not been conducted since January 29th, 2025.

During the follow up inspection, a caregiver left 6 children unsupervised in the pre-kindergarten classroom.

During the follow up inspection a caregiver yelled at children to sit down and stop running.

A caregivers background check was inactivated on 8/19/25 and did not have an active and eligible background check during the inspection. The operation has not received a new eligibility determination letter from CBCU stating the caregiver can be present. Note: The caregiver left the operation during the inspection.

The employee left in charge did not have access to children's files that were requested during the inspection. Note: The operation provided the childrern's files when the owner arrived.

Based on the information obtained, there was sufficient to support that a caregiver has yelled at children.

Staff files evaluated were missing the following information: - Proof of meeting the minimum education requirements - Proof of the Background Check Request (Note-these were printed and placed in the file during inspection)

An activity schedule was not posted in the Pre-K 2 Classroom during the inspection.

2 children were not signed in to the daycare's tracking system during the inspection. Note: The operation signed the 2 children in during the inspection.

Documentation showing current CPR and First Aid training certificates for 2 out of 10 staff files evaluated were not available to review upon request during the inspection.

A child was climbing on the nap mats and a caregiver put the child in a restrictive device.

2 out of 10 children's files evaluated were missing the address for the emergency contact person.

The tarp under the loose-fill was showing and it could cause a tripping hazard.

There was no health inspection available for review during the monitoring.

There were menu available during the inspection.

The emergency practices was missing 3 months of documentation.

There were no playground monthly check list available for review during the inspeciton.

There were no updated feeding instruction for an 11mos. infant.

During the walkthrough three infants were on individual cots, beds or mats. Note: This was corrected at the inspection.

During the inspection, two caregivers files was not available for review during business hours.
