Another caregiver who signed in 8/29-8/31 and caregivers for toddlers did not have an active background check submitted or eligible results on file.

I observed 3 infants in 3 different classrooms sleeping in a crib with a pacifier in the mouth with the attachment adhered.

The operation did not document/conduct a fire drills as required per minimum standard guidelines

A caregiver with a rehire date of 10/25/21 was present with children but did not have a background check submitted or eligible results on file. This was corrected at inspection when the caregiver left the operation and will not return until eligible results are received.

A licensing incident report was not created for a child who received an injury while in care which required medical attention.

Testing data for the impact rating of the unitary surfacing was unavailable for review at the time of inspection.

A child was injured while in care and the parent was not informed prior to the child leaving care.
