Based on the information obtained, children were observed running in and out of the restroom unattended.

During the inspection, I observed one classroom with part of the ceiling open. I also observed another classroom with water damages to the wall and water spots on the ceiling.

During a DFPS investigation, it was determined that a caregiver yelled at children in care.

Based on the information obtained during this investigation, the operation will be cited for a caregiver who used prohibited punishment towards a child in care.

During the initial inspection a person who calls themselves the center director did not have a background check on file while present at the operation and around children in care.

The operation failed to designate a qualified director.
