It was observed in the infant classroom that ten of ten cribs present had loose fitted sheets on the mattresses.

It was observed in one of four classroom was using screentime during naptime/resttime.

The last fire inspection conducted was 1/6/2025.

In one classroom a caregiver was relieving another staff member for a break and did not have documentation in the classroom showing each child in the classroom's age.

The operation did not have medication logs available for review. In one classroom for two children, observed a staff member has documented the name of the medication and the time given on an infant daily report which is sent home to the parent.

Two medications in one classroom did not have the complete name of the child in which the medication belonged to. This was corrected at inspection as staff wrote the name of the children on the medication. In another room not used by children where medication is kept there was one medication that was not labeled with the child's name for whom the medication is for. All three medications did not have the date the medication was brought to the operation.

In one classroom the operation reduced the ratio during nap time, There were 27 children in the room with a specified age of 3 years being supervised by one caregiver. There was not another individual present in the building who was not supervising children.

A 10 month old was observed wearing a teething necklace. This was corrected at inspection as staff removed the necklace from the infant.

The operation did not have documentation of fire drills conducted nor documentation of fire extinquisher checks conducted on file for review by Child Care Regulation.

In one classroom there were 14 children, ages 12 months to 38 months old with two caregivers. You may combine infants with older children as long as the oldest child in the group is not more than 18 months older than the youngest child.

One staff members record lacked documentation of their date of hire, documentation of their education qualification, employee statement regarding center and personnel policies, an orientation statement regarding abuse/neglect training, a notarized affidavit, and a completed Form 2912, Pre-Employment Affidavit for Applicants for Employment at Certain Child Care Operations. Three other employees personnel record lacked a completed Form 2912, Pre-Employment Affidavit for Applicants for Employment at Certain Child Care Operations.

A sampling of children records were evaluated. Four children records lacked the child's date of admission. Two children records lacked the address of the children's designated contact. One child's record lacked the phone number of the school the child attends. Two children's immunization record was observed incomplete. One child's record lacked the phone number and address of their doctor. One child's record lacked documentation of emergency care authorization.

At inspection there were two classrooms of children napping. Two caregiver were supervising 12 children, ages 14 months to 3 years (38 months) and in another room one caregiver was supervising 29 children, ages 2 years to 5 years. There were no additional staff in the building.

One employees record lacked documentation of completion of 8 hours pre-service training before being counted in the child/caregiver ratio.

The use zone is not being met around the musical play equipment on the older children's playground.

The Emergency Preparedness Plan was evaluated and lacked the following written procedures: 1) How your staff will continue to care for the children until each child has been released; and 2) How you will reunify the children with their parents as the evacuation, relocation, or sheltering/lock-down is lifted.

One caregiver lacked documentation of completion of first aid training.

Three children left their classroom and were found in a empty classroom during inspection.

The operation had not certified in writing that they reviewed the CPSC recall list and that there were no recalled items in the center. You may view all current and past recalls through the CPSC s Internet website at: www.cpsc.gov.

The operational policies were evaluated and lacked the following written procedures: 1) procedures for conducting health checks; 2) The benefits of physical activity and outdoor play; 3) The type of physical activity (structured and unstructured) that children may engage in at your operation; 4) The criteria you will use to determine when extreme weather conditions pose a significant health risk that prohibits or limits outdoor play; 5) Two children records evaluated did not have signed documentation of the parents rights.

In the infant/toddler room the caregiver diapers children in the floor of the classroom on a changing mat.

The caregiver lacked documentation of completion of 24 hours pre-service training.
