This standard was tasked as a result of a DFPS investigation and found to be deficient. A child left the playground unsupervised and was alone in the parking lot for five minutes.

The operation has not had monthly fire drills. The last documented drill was in April 2026.

This standard was pulled as part of an investigation and found deficient. A caregiver was observed on surveillance hitting on child and pulling the child up by their arm in an inappropriate manner to avoid being stepped on by other children in the class.

Equipment that was holding water was not dumped before children went outside. This was corrected at inspection when the water was dumped out.

The electronic safety alarm in one vehicle has not properly worked in at least 12 months.

Caregivers in one classroom are unaware of a daily schedule for the room. A daily schedule was not posted in the classroom.

One classroom was observed out of ratio during the inspection. The SAG of the class was three years old. There were 17 children with one caregiver. The operation was over ratio by two.

One Cargiver file reviewed only had 14 of the 24 required annual training hours.

When reviewing the directors file, only 25 of the 30 required annual training hours were available to review.

Four children files reviewed did not have the required information needed for emergency care.

This standard was evaluated as part of an investigation and was in deficient. Video surveillance provided showed a caregiver had pushed a child in the face, thumping children on the ear, and forcing them to put their head on the table.
