A non-walking infant was sleeping on a mat. The Director provided compliant play yards. The infant was placed on their back in the play yard, during the inspection.

The director or person in charge was not available. A designated individual was not assigned in the absence of the person in charge. The director arrived during the inspection.

One classroom had uncovered outlets in reach of the children in the group. During the inspection, the director covered the outlets in the classroom.

A classroom was left unattended without a caregiver present.

The operation has a staff member who is present at the operation without a background check.

Several classrooms in the operation had classrooms that didn't have adequate lighting during naptime not allowing for proper supervision.

The operation has several caregivers without documentation to show education qualifications.

There are children enrolled without immunization records on file.

Several classrooms have uncovered outlets leaving them exposed to children.

One child with diagnosed food allergies does not have a parent signature for the food allergy emergency plan.

Drill start times are documented, however the length of each drill is not.

Expired medication is maintained for a child with diagnosed food allergies.

Personnel files were missing Affidavits.

One file was found to be missing an emergency contact.

There were two files missing authorization to obtain emergency medical care.

It was determined that the operation was subject to regulation. There were eighteen children under the age of three years old in care during the inspection. The director stated that this age group of children attends three or more days a week.
