The operation was observed to have multiple ceiling tiles with stains and needing replacement. Multiple vents where observed to have dust covering the vents. Lights where observed to be missing covers as well as those that had covers where seen with visible dust.

6 employee files were reviewed. 3 employees did not have proper first aid and CPR training.

Two children that are no longer in care had medication present. This was corrected at inspection when the director removed the medications from the box and contacted family to return.

Two children are missing their Health Statements.

One child was missing parent's authorization to administer medication.

During the walkthrough inspection areas were observed to be dirty or unsanitary that could attract pests. Thorough cleaning and sanitizing is required to ensure a healthy environment for children.

The operation was not following the written activity plan by not going outside.

The operation did not notify CCR of a reduction in space.

The fire inspection was conducted on 6/20/2024 and was failed. Corrections have not been made at this point.

A current fire inspection was not available for review at the time of the inspection.

I observed a play yard to have lose fitted sheets.

I observed a caregiver not wash their hands after removing gloves following a diaper change.

I observed an excess of trash on the floor of the transportation bus.

I observed children not wash hands after using the restroom/after a diaper change in 3 classrooms.

A gas inspection was not available for review at the time of my inspection.

A caregiver had unsupervised access to children prior to completing first aid training.

I observed the pea gravel to be at approximately 1-2 inches under the rocking equipment.
