During the inspection, a caregiver was found caring for a group of 11 children. The specified age of the group of children required 1 caregiver for every 9 children. The Director had more caregivers arrive, bringing the operation back into ratio by disperising the children to other rooms.

An outlet accessible to children was uncovered in the pre-k aged classroom. The caregiver covered the outlet with an outlet cover when this was brought to her attention.

During the inspection, a 6-month old child was observed sleeping with a blanket in the crib. The Director then removed the blanket.

During the inspection, Lysol toilet bowl cleaner was located next to the children's toilet in the three to five year old classroom.

During an inspection, a classroom was observed to have five toddlers in care. There was not a caregiver in the classroom during the walkthrough.

In the documentation completed as part of a DFPS investigation, it was found that the Director did not ensure that the facility was operating in compliance with minimum standards. On multiple occasions, children were not appropriately supervised, parents were not notified of serious incidents, and self-reports were not made.

In the documentation completed as part of a DFPS investigation, it was found that an infant child was left unsupervised in the operation office. Operation staff did not inform the parents of the infant that the infant had been left unsupervised.

In the documentation completed as part of a DFPS investigation, that the operation did not report multiple instances of unsupervised children to CCR.

In the documentation completed as part of a DFPS investigation, it was found that an unsupervised infant child was injured by older children who were also unsupervised while in care at the operation. The infant child required medical attention for the injuries that the received.

In the documentation completed as part of a DFPS investigation, it was found that an infant child was left unsupervised in the operation office. Additionally, two three-year-old children left a classroom without caregivers being aware of them being missing for 6-7 minutes.

This standard was evaluated as part of an investigation and determined to be deficient. It was determined that the operation was transporting children in care without notifying CCR that transportation was a service that was offered. CCR notification requirements were discussed with the operation.
