One caregiver was observed assisting two children with toileting at one time. The caregiver did not change gloves and wash hands in between. Based on information gathered during inspection, caregivers have not been washing their hands as required by minimum standards.

A three year old child was observed exiting the Three's classroom alone and walking to the indoor play area. The teacher was observed diapering children when the child exited the room.

Two children do not have immnization records on file.

Four children are do not have food allergy plans signed by parent and health care professional.

One caregiver was observed outside caring for 16 children with a specified age group of three years old.

The operation has not had a fire inspection since 9/2019.

Three caregivers did not know/were unsure of how many children were in care in their classroom during inspection.

Based on the evidence gathered through the course of investigation. A caregiver was observed forcibly grabbing three children by the arm and dragging them to different areas of the classroom. The caregiver was also observed forcibly grabbing another child by the arm. The caregiver was also observed forcibly sitting a child in a chair and forcing the chair under the table.

Based on information gathered through the course of investigation, children in the Three year old classroom were observed not washing their hands after outdoor activities, before eating, and after toileting.

It was determined through the course of an investigation that the operation the operations staff did not use good judgment when knowingly dismissing a child who's under clothes were soiled.

During the course of investigation it was determined that a caregiver left children unsupervised in the classroom multiple times in a 30 minute period. It was also determined that the caregiver allowed a child to exit the classroom onto the playground alone to retrieve an personal item.
