Upon arrival there were three children under the age of two watching the television while eating breakfast. The specific age is 18-23 months. The director and caregiver turned the television off and removed the children from the eating area.

During the monitoring visit, the infant cribs were not labeled with children's names or a number system. The caregiver labeled the cribs during the visit.

This standard was evaluated with regard to an investigation, and a violation was found. It was determined through interviews that a caregiver and employee staff have physically disciplined their child while the child is in care at the operation.

During the follow-up visit, there was one caregiver who did not have a background check while supervising children n a classroom. The caregiver was not able to leave due to the operation being out of ratio.

During the monitoring visit, some of the mats and the cots were not labeled in two classrooms during nap time.

During the monitoring visit, there was a wasp nest under an outside covered area. The wasp nest was located in the corner of the covered outdoor space.

During the monitoring visit, the operation did not have the emergency medical transportation information for children that are transported on the operation transportation vehicle.

During the monitoring visit, the operation did not have an annual fire inspection available for review during the monitoring visit.

Five out of ten staff did not have the required annual trainings for emergency preparedness, preventing and controlling the spread of communicable disease, food allergies, building safety, and handling and storing disposable objects.

The operation did not have the inclusion policy in the parent handbook.

The director and two additional drivers did not have the transportation training certificate available for review during the monitoring visit.

Five out of ten staff files did not have the affidavit form notarized.

Six infants did not have a written feeding plan available for review during the monitoring visit.

The cleaning products in the infant classroom, toddler classroom, and preschool 3 classroom were accessible to children. The items were in a cabinet without locks and on top of a shelf near the restroom in the preschool classroom.

Eight out of ten children did not have the pre-service orientation training in the files available for review during the monitoring visit.

Two out of ten children did not have the emergency care authorization section filled out on the admission form.

One of the transportation vehicles did not have a fire extinguisher in the vehicle.

Six out of ten children did not have the health statement in the files during the monitoring visit.

The preschool 3/4 classroom had nineteen children with one caregiver. The specific age is four years old. The children were playing with various toys on the floor with the caregiver. The operation corrected the ratio during the visit by adding another caregiver to the classroom. The class was in ratio with two caregivers and nineteen children.

The cribs in the infant room had loose-fitting sheets. During the monitoring visit, the caregivers added bed sheet safety straps to all the cribs for infants in the classroom.

There were three infants in the restrictive devices from 12:17 to 12:45 pm. The caregiver removed one infant from the device at 12:39 pm., and the other two remained until 12:45 pm. The caregivers sat on the floor with the infants with small toys to play with. The ages were 7 months, 7 months, and 13 months.

The operation allowed a 14 year old volunteer to be present at the operation without a background check. The 14 year old was observed in the classroom with access to the children. This was corrected at inspection by having the 14 year old leave the operation.

After the caregiver helped the 2 year old children in the bathroom with toileting, the caregiver washed her hands but the children did not wash their hands.

The water hose was left outside on the toddler playground. 1 child was observed picking up the water hose.

Upon arrival the TV was on at the operation for the school age kids. The TV was still on after 1 hour, playing Mr. [name redacted] Out.

This standard was evaluated during the investigation. It was determined that the operation was out of ratio in the 2's classroom, leaving 1 caregiver with 17 2-year-olds at once.
