One of six children did not have a health care professional signature on file after child has been enrolled form more than 12 months at the center.

An employee contacted a former employee to call an agency on behalf of the daycare.

A bottle was propped on a blanket to feed an infant who could not hold bottle. This was corrected when I informed caregiver and she removed bottle and prop.

Two of six children did not have a health statement on file within one week of attending daycare.

One of six children did not have immunization records on file availble for review.

4 children were present on 08/14/25 and none had documentation of diaper changes on infant daily report. This was corrected at inspection when caregiver changed a diaper and documented time.

Three of four children did not get signed out on 08/14/25.

The director did not have annual training records availble for review.

Two of six children did not have signature or date by parent/guardian on admission form giving permission for medical emergencies.

The daycare had an unlocked container that read "keep out of the reach of children" stored under the sink in the restroom while a child was using the facility. This was corrected at inspection when daycare removed chemicals from restroom.

An uncapped medication was stored in the same refrigerator as food at the daycare. This was corrected at inspection when the center threw away medication that was in refrigerator.

3 of 6 children did not have signature or date for authorization for medical treatment.

1 child did not have signature, date or a check on any selection option on admission form.

1 child was observed eating a cracker and putting cracker on table. No plate or napkin was provided to child at time of serving. This was corrected at inspection when caregiver provided child with another cracker and paper plate.

The director was not present when inspector arrived and could not provided documentation to show she has been present 75 percent of time or at least 30 hours. This was corrected at inspection when director started documenting her time for current week.

2 children were sharing a crib. They were utilizing the same crib at different times. 7 cribs were observed in infant room with 8 children under the age of 12 months present.

Child Product Certification had not been done within the past 12 months.

1 van used to transport children was missing a first aid kit.

A child was laying down on carpet with a bottle propped. This was corrected at inspection when caregiver picked up child to feed infant.

A child was laid down to rest in a crib that was labelled for another child. This was corrected at inspection when caregiver changed the sheet in crib.
