A CPR/First aid training was not available for review. Director could not find the training in the office.

The transportation vehicle used to transport children in care did not have a child safety alarm. The vehicle was purchased/leased after December 31, 2013, and is designated to seat eight or more people.

A transportation vehicle used to transport children in care did not have a fire extinguisher.

There was no Emergency Medical Information, parent's name, telephone numbers and emergency telephone for a child being transported from school to the operation. Correction at Inspection: The information was placed in the transportation binder.

The Infant Daily Report is missing the Infant's Mood and Summary of Activities.

The outdoor play area was observed with two exposed nails.

Two children were observed not securely fastened in their highchairs, as they were only restrained by a single leg.

The outdoor play area was observed with overgrown weeds and an exposed nail.

Based on the information gathered during the investigation, it was determined that a caregiver was witnessed yelling at the children and forcibly managing their physical placement to ensure they occupied their seats.

Caregivers are not consistently facilitating outdoor play for the children on a daily basis.

Three caregivers did not have the following annual training topics: -Child maltreatment (1)Recognizing and preventing shaken baby syndrome and abusive head trauma (2)Understanding and using safe sleep practices and preventing sudden infant death syndrome (SIDS) (3)Understanding early childhood brain development. (1)Emergency preparedness (2)Preventing and controlling the spread of communicable diseases, including immunizations ; (3)Administering medication (4)Preventing and responding to emergencies due to food or an allergic reaction (5)Understanding building and physical premises safety (6)Handling, storing, and disposing of hazardous materials

There were no Emergency medical transport and treatment authorization forms for two children who are transported from school. Corrected at Inspection: The forms were placed in the transportation binder.

A child did not wash their hands after restroom use. Note: The caregiver was advised to direct the child to wash their hands.

The fire inspection report was not available for review. Staff training records were not available for review.

There was a gap on the bottom of the wooden fence because it was missing a horizontal wooden panel. There were exposed nails on the wooden fence which pose a safety hazard for the children during outdoor play.

Three infants were not strapped in their highchairs. Corrected at Inspection: The infants were immediately strapped during the inspection.

Operation did not have a plan to handle transportation emergencies.

Feeding instructions for 2 infants have not been updated. Feeding instructions for 1 infant was not available for review.

Childrens product certification was last completed on 1/10/2023.

Classroom was observed to not have enough toys for all of the children. Children were observed fighting with each other for toys.

Two-month-old infant was observed swaddled in a crib.

No drinking water was available for toddler and school-age children during mealtime and during active play.

Children's hands were not washed after diaper change.

Caregiver was observed not washing her hands after changing children's diapers.

Two classrooms did not have an activity plan.

A caregiver left three children unattended in the after school room when the caregiver stepped out of the room to take a child to the toddler room.

Two school aged children in the afterschool room were observed watching photos and videos on a cell phone. The television in the afterschool room was on playing a cartoon not related to the planned activities. The cartoon playing on the television was (Masha and the Bear).

The operation did not notify child care regulation after learning about a child being injured while in care and requiring medical treatment.

Based on preponderance of evidence gathered during the investigation, two caregivers have yelled at children in care at the operation.

A 6-month-old infant had a blanket inside their crib during nap time. Infant was awake. NOTE: The blanket was removed by caregiver upon my arrival to the room.

Child/Caregiver Ratio was deficient in the infant classroom by 1 child. There were 2 caregivers with 11 children ages 1 month to 20 months old. Specified Age Group was 0-11 months old. NOTE: This was corrected by the oldest child being moved to another group. Child/Caregiver Ratio was met.

One infant was observed in the same group of children as an 11 year old child. In another classroom, a 20-month-old was observed in the same group as a 1-month-old infant. 18-month difference was exceeded. NOTE: This was corrected during the inspection. Children were regrouped and compliance was met.

A new caregiver having unsupervised access to children in care did not have First Aid training.

Two infants were observed in the same group of children as a four year old child. 18-month different was exceeded. NOTE: This was corrected during the inspection. Children were regrouped and compliance was met.

Notarized Affidavit for two caregivers were not available for review.

Two activity plans posted in two classrooms in use were not dated.

First aid training for infant caregiver was not completed prior to unsupervised access to children in care.

Documentation of Pre-service training completion was not available for review for an employee counted in child/caregiver ratio.

Fire drill documentation for the months of June and July 2022 showed that the exit from the building was in over three minutes. Seven and six minutes respectively.

An infant was observed watching a music video on a tablet. NOTE: This was corrected during inspection. Tablet was put away by caregiver.
