A four-month-old infant was observed asleep on a bouncer. The infant was moved into a crib after being instructed to do so.

There was no mark to indicate the depth of the loose-fill surfacing material, mulch, for a slide structure.

One personnel record was missing the education document, and form 2912 for a caregiver hired on 5/04/26. Training records were not available for one employee.

The fire extinguisher was not secured in the transportation vehicle. There was no Emergency medical transport and treatment authorization form for one child being transported from school. Corrected at Inspection: The missing information was placed in the transportation binder.

The admission documentation for one child was incomplete due to the absence of certain pages.

The classrooms utilized by the children were observed with water stains on the ceiling tiles and dead insects in the light panels. The outdoor area was observed with two exposed nails in the wooden surroundings of a tree. A trash bag was in left in the infant room. Note: The trash bag was removed. There was loose ceiling vent in a classroom used by the children.

Three caregivers completed Pediatric CPR training online without hands-on skill-based instruction, and practice.

There was no Food Allergy Emergency Plan for two children with a food allergy.

Director did not have a current transportation training.

Loose fill surfacing (mulch) was not maintained at the proper depth for two play structures in the outdoor play area. The dirt underneath the mulch visible and over grown weeds were observed within the mulch.

5 out of 12 staff records reviewed are missing pre-service training topics.

1 out of 12 staff records reviewed is missing a valid high school diploma or equivalent. Discussed waiver/varience with director.

The investigation shows that a staff member started working at the operation before there was a cleared background check. (NOTE: Employee is no longer working at the operation.)

CPR training taken by 2 of the caregivers present during inspection did not include use of a CPR manikin and hands-on skill-based instruction.

The operation did not inform Child Care Regulation of a child injury that required medical treatment.

A portable basketball hoop within reach of children was observed tilted and without sand or water to sustain the base. It was located in the playground area where children were playing. NOTE: This was corrected during inspection. Equipment was removed from the child care area during inspection.

Orientation statements of 4 out of 10 personnel records reviewed were not available for review during inspection.

Diploma of education outside of U.S. was not translated and equivalence information was not available for review for 1 caregiver.

Updated immunization records of 2 out of 10 child records reviewed were not available during inspection. NOTE: One of the immunization records was requested from parent and made available during the inspection.

Hand washing did not occur before snack in the Pre-K classroom and before dinner for a group of children in the School Age classroom as observed during inspection.

Employee in the process of receiving high school equivalency was providing care of children during inspection. Operation has not submitted a variance for employee.

A personnel record was not available for review.

The grass is overgrown in the playground. A board on the fence is loose.

The loose fill underneath a slide which measures less than 5 ft in height is not at the required 6 inches.

The class was not following a system to say which side would face the ground and which side would face up.

During the investigation it was found that children were placed in high chairs as a form of punishment. The caregiver is no longer employed at the operation.

A caregiver at the operation has a high school diploma which is not valid.
