The child care operation did not have a current fire inspection.

A caregiver unintentionally pulled a child's arm, resulting in an injury to the child.

Six infants did not have a current infant feeding instructions.

The operation failed to submit the number of employees who left employment in 2024 by 1/15/25.

There are 12 cribs observed with chipped paint.

Based on the information obtained during the course of the investigation, the director failed to notify licensing of extended leave.

Soiled diapers were observed in three different trash cans without a cover in a children's restroom. Note: This was CORRECTED during the inspection as covers were placed on the trash cans.

The diaper changing mat in one of the toddler rooms was torn.

Most recent fire inspection provided is dated 4/26/2023 and a renewal inspection is due every twelve months.

Fire extinguishers were not currently serviced. The last service date was on March 2023.

A child's record reviewed was missing the immunization documentation. Note: This was CORRECTED during the inspection as the child's immunization records were obtained.

A caregiver counted in the child/caregiver ratio was missing 18 clock hours of pre-service training.

Two caregivers with education received outside of the United States did not have the education documentation translated in English.

Two infants were observed sitting at a table without the safety straps fastened.

Two caregiver's were missing the following information on their records: A caregiver with DOE 8/28/2023 was missing a notarized Licensing Affidavit for Applicants for Employment form, A statement signed and dated by the employee showing he has received a copy of the child-care centers: (A) Operational policies; and (B) Personnel policies; and A statement signed and dated by the employee verifying the date the employee attended training during orientation. A second employee with a DOE 8/14/2023 was missing A statement signed and dated by the employee showing he has received a copy of the child-care centers: (A) Operational policies; and (B) Personnel policies; and A statement signed and dated by the employee verifying the date the employee attended training during orientation.

There was no file available for a child in care.

A caregiver was observed caring for 7 infants. The ages of the children in the group were two 7 month olds, a 10 month old, two 11 month olds, a 13 month old, and a 15 month old. According to the ratio chart the maximum number of children one caregiver may supervise is 5. The group was out of ratio by two children. Not: A second caregiver stepped into the room during the inspection.

A caregiver caring for a group of children did not have training in pediatric firs aid.

The lighting in an infant room was not adequate to allow visual supervision of all children in the group at all times during naptime.

There was more than an 18 month difference when a 16 month old infant was combined with toddler and pre-k children during naptime. The oldest child in thee group was 3 years old and the youngest child was 16 months old.

Two infants sitting at a table were provided screen time as they were watching a video (Ms. [name redacted]) on a tablet device during nap time. Note:This was addressed with the caregivers in the infant room and a caregiver turned off and removed the electronic device from the table.

No use of screen-time activities was observed in any of the groups of children present during inspection. However, a photograph of a child sent to a parent revels that a child was participating in screen time activities while having a snack on 8/11/2023 at around 1:08pm.

Based on evidence gathered during the investigation, the operation allowed an individual without a background check to be in contact with children in the infant classroom on 8/4/2023.

A Pre-K child was observed playing a video game on a phone/tablet device during nap time. NOTE: This was corrected. Caregiver placed a mat for the child to rest and electronic device was removed.

Child/Caregiver Ratio was deficient during the inspection as 1 caregiver was left alone with 9 infants with a Specified Age Group of 0-11 months old. NOTE: This was corrected during the inspection by a second caregiver joining the classroom and ratio being met.

An individual with a pending background check was observed providing care of children. A second individual without a background check was inside a classroom with children. NOTE: This was corrected during the inspection by both individuals leaving the premises and qualified caregivers joining each classroom.

Caregiver in the toddler classroom did not wash hands after completing a diaper change on a child.

Operational policies were incomplete for Promotion of indoor and outdoor physical activity.
