A van used to transport children did not have a working child safety alarm during inspection.

While checking records, two caregivers, who have worked at the operation for more than a year, do not have current CPR training. Both caregiver's CPR training expired 02/2025.

In review of records, the director, who provides transportation to children in care, does not have a current proof of transportation training available for review. Training certificate available is dated 02/2022.

While reviewing records, one caregiver did not have proof of training in first-aid available for review,

While reviewing records, three caregivers do not have proof of training in food allergies and hazardous material. Two caregivers, who are responsible for administering medication, do not have proof of training in administering medication available for review.

A child in care sustained an injury that required medical treatment by a health-care professional. The operation was made aware that the child received medical treatment on the same day and did not notify CCR within two days as required.

During an investigation it was found that four children ranging in age from 8-11 years were not adequately supervised in the outdoor play area or while in transport to the operation. The caregivers responsible for these children were not within physical proximity of the children and did not have auditory or visual awareness of the children.

During nap time, mats/cots were too close together and did not allow for sufficient walk space in between them. Photos taken. This was corrected at inspection when staff rearranged the mats/cots to provide sufficient space in between them. Photos taken of correction.

Two children's records are missing a complete and current health care statement. Photos taken.

During inspection the cots assigned to children in care had a number labeled on them but were not associated with the children napping on them. This was corrected when the director made a list and assigned the number to children in care.

During review of records one employee did not have an affidavit on file.

During inspection a gas leak test had not been conducted; the last test completed was dated was on 7/2/2019.

A time of inspection the director indicated that the safety alarm was not being used because it was not working or on silent. This was corrected when the owner of the center fixed the alarm and it was functioning properly.
