In review of records, the director did not have all of the required 30 hours of annual training available for review.

Operation does not have a playground maintenance checklist available for review, showing the damage observed to an outdoor play structure prior to children using it. Children observed using play structure with sheer points. This was corrected at inspection when caregiver completed a playground checklist showing damaged equipment.

In review of caregiver trainings, two did not have 24 hours of annual training.

The child-care center director or designee did not ensure hazards, shear points, on an outdoor play structure were repaired or removed prior to being used by children. This was corrected at inspection when designee put tape over shear points of play structure.

One driver at this operation did not have a current transportation training certififcate on file.

During inspection, two caregivers were observed to be present without cleared background checks, to include pending fingerprints and pending national sex offender registry checks. One caregiver was supervising the kindergarten classroom, and the other caregiver was supervising the first grade classroom. The caregivers were allowed to be present before the operation received notification from the criminal background check unit that the caregiver was eligible. This was corrected when the caregivers were removed from the operation.

The operation did not have a current gas leak inspection on file. It had expired on 6/12/22.

A caregiver failed to use a positive method of discipline when trying to redirect a child, causing a scratch to the child's neck.

The operation does not a carbon monoxide detector available for review.

Three transportation binders were evaluated there was 3 missing forms that contain the child's emergency medical and e emergecy contact information. One child's information was noted to be located another binder that belonged to a different route. This was corrected during the inspection by printing and adding the admission forms in the binders.
