Operation completed but did not have documentation showing a fire drill was done in December 2025. This was corrected at inspection when a caregiver documented a on an emergency practices form showing a fired drill was completed 12/18/2025.

During a DFPS investigation visit to the operation, some children were observed watching YouTube without the caregiver monitoring the screen time.

A toddler was observed walking around while holding and drinking from a training/sippy cup.

There are two metal shelves that were not anchored to the wall or the ground in the outdoor play area for after-school aged children. These metal structures were not being utilized for storage but were empty. These metal structures could tip over if climbed on, posing a risk of harm to children in care.

Ten children's records were reviewed. Six of the ten children's records were incomplete; there was a combination of Health statements, admission dates and emergency care authorizations missing among these six records.

The operation did not have fire drills posted or available for review. CCR was not able to verify when the most recent fire drill was conducted.

All of the caregiver's records that were reviewed were missing the required 24 hours of annual training. Director stated the staff had not yet completed the required training.

While the operation had multiple first aid kits in different rooms/areas, none of them were complete. There was a combination of missing items among the various first aid kits to include missing thermometers, adhesive tape, scissors and band aids.

In the infant room, the cribs were positioned in a way that allowed for the children to reach into the crib of another. This was corrected at inspection when the caregiver repositioned the cribs so the children could not reach and touch each other.

The operation has a fire inspection that expired on 03/15/2023.

The director and the one provider did not complete the transportation training by the initial compliance date of 10/31/22. A new compliance date of 11/21/22 was given to the operation to comply.
