In review of records, two children's files did not have signed health statements available for review.

In review of records for HASA, Primary Caregiver does not have proof of training in handling, storing, and disposing of hazardous materials, administering medication or how to respond to food allergies.

Operation did not have a current consumer prodcuct certification. The last time the form was completed was 01/16/2024. This was corrected at inspection when primary caregiver looked at the consumer product safety website for recalls on household and daycare products.

Caterpillar play structure in the outdoor play area has two cracks at the top, leaving a sheared and pinch hazard exposed to children. This can cause lacerations or abrasions to a child in care. This was corrected at inspection when caregiver placed duct tape over the sheared areas.

During inspection, the operation was observed to be over ratio. Specifically, primary caregiver was observed supervising a total of seven children ages 7 months, 14 months, 14 months, 14 months, 2 years, 2 years, and 2 years. The most number of children one caregiver may supervise when having four children between the ages of 0-17 months is four children plus two additional children ages 18 months through 3 years. Therefore, the operation was over ratio by one child. This was corrected at inspection after one infant child was picked up by a parent.

The operation does not have a current annual verification of liability insurance. The operation has a next annual verification past due date of 5/29/22. This was corrected at inspection after Director/primary caregiver submitted a Verification of Liability Insurance form.

An outdoor climbing equipment is placed over a concrete slab. The equipment will need to be placed over to grass area, or have unitary surfacing placed under the equipment.

Based on an audio recording evaluated during the course of the investigation, it was determined that the provider did not comfort a child in care that was crying for over 5 minutes.

Based on an audio recording evaluated during the course of the investigation, it was determined that the provider used harsh language while children were in care.

Based on an audio recording evaluated during the course of the investigation, it was determined that the provider is heard yelling at the children in care.

Based on an audio recording evaluated during the course of the investigation, it was determined that the provider did not demonstrate self control while caring for children by yelling at children, talking harshly at children and using verbal threats towards children in care.
