In review of the director's file, director had 23 of the required 30 hours of training.

A van used for transporting children has a fire extinguisher that has not been serviced.

In the infant room, after changing a child's diaper, caregiver did not wipe off the child's hands or wash the child's hands.

During diaper changes in the infant room, a caregiver did not wash her hands after removing the gloves in between diaper changes.

While unloading children from a transportation van at the operation, a caregiver/ driver disarmed the child safety alarm before conducting a walk through and physical check of the vehicle for children.

In review of employee records, one employee is missing 15 hours of annual training.

Operation did not have a documentation showing proof of a current gas leak inspection. Last inspection done 09/21/21 expired 09/21/23.

In the infant room, two children's bottles were not labeled. This was corrected at inspection when the director labeled the bottles with the children's names.

One child's admission form was not signed by parent.

Director is missing a total of ten hours of annual training from the required 30 hours.

During inspection, in the outdoor play area, the operation did not have the required 6 inches of loose fill surfacing material under a climbing/rocking/moving/bouncing metal jeep spring rider equipment. The operation only had about 1-2 inches of loose fill. The equipment's highest designated play surface is approximately 1ft.

One child's file was missing a completed and signed authorization to obtain emergency medical care.

During inspection, the operation was missing the required posting/licensing notice Keeping Children Safe. This was corrected at inspection after inspector provided the operation with a copy of the notice and the notice was posted.

The operation will need to pay an outstanding background check.

The transportation vehilce will need to have a first aid kit available in the vehilce during all transportation routes.

The transportation vehicle does not have a required booster seat for child that was tranported on this day and requires this type of equipment.

The transportation vehicle will need to have a fire extinguisher installed in the transportation vehicle.

Three cribs in the infant room do not have documentation that they meet the new CPSC requirements.

The outdoor ramp has a piece of wood missing at the base of the walking ramp. This poses a tripping hazard for children and staff. --------------------------------------------------------------- An outdoor exit fence door will need to have a latch installed. The door is not able to be closed and a child in care can exit through this door. -------------------------------------------------------------- The carpet placed over the ramp leading to the outdoor play area will need to be anchored down. This carpet has torn and is rolling up causing a tripping hazard.

The emergency authorization information for one child being transported will need to be included in the transportation binder.
