The latch on the gate that was installed to prevent access to the pool was within reach of the children in care.

The children in care were watching a television show, and one of the children was ten months old. This was corrected during the inspection when the television was turned off.

At the time of the follow-up inspection, the primary caregiver was still missing three instructor-led training hours and the following training topics: emergency preparedness, food allergies, building and physical premises safety, handling hazardous materials, and preventing communicable diseases.

A substitute caregiver was allowed to watch children alone at the operation before the primary caregiver received notification of whether or not the substitute caregiver was eligible to present. This was corrected during the inspection when the children in care were sent home, and the substitute caregiver left the operation.

One of the children who was present was not enrolled and did not have any admission information.

At the time of the follow-up inspection, the primary caregiver had not submitted the required staff records for the substitute caregiver.

The substitute caregiver who was in charge during the primary caregiver's absence did not have phone numbers for some of the parents of the children in care.

The primary caregiver allowed an individual with an ineligible background check to be present at the operation. This was corrected during the inspection when the individual left the operation.

An infant under twelve months old was observed sleeping in a play yard while wearing an amber teething necklace. This was corrected during the inspection when the necklace was removed and stored in the infant's bag.

An infant was observed sleeping in a play yard with a loose-fitting sheet that was coming off of the play yard mattress.

A caregiver without a background check was present during the inspection and was caring for children alone while the primary caregiver was out running errands. This was corrected during the inspection when primary caregiver returned and the caregiver without a background check left the operation.

An infant under twelve months old was observed sleeping with a blanket in the play yard. This was corrected during the inspection when the blanket was removed.

An infant was observed sleeping with a bottle in a play yard. This was corrected during the inspection when the bottle was removed.

One of the children present was not enrolled and did not have any admission information.

The primary caregiver stated that one of the infants has been sleeping in a swing instead of a play yard. This was corrected during the inspection when the primary caregiver was informed that infants cannot sleep in swings without a sleep expection form signed by a doctor.

The parent handbook was missing information on parent's rights and procedures for medical emergencies.

There was an assistant caregiver present who did not have a background check. This was corrected during the inspection when the caregiver left the operation.

The bottles for the two infants in care were not labeled.

The children's product certification form was last signed on 10/18/24.

The afterschool room did not have a smoke detector.

The two infants in care were still on formula and did not have written feeding instructions.

The primary caregiver's first aid and CPR expired on 04/25/2026 and she did not have proof of renewed first aid and CPR training.

Several outlets were missing outlet covers.

An infant was observed sleeping on a boppy pillow on the floor. This was corrected during the inspection when the infant woke up, and the primary caregiver was also informed that infants must sleep in their own play yard or crib,

Out of four children's records reviewed, two were missing everything except an immunization record, one was missing emergency consent to medical treatment and an admission date, and one was missing parental acknowledgement of receiving the parent handbook.

An infant was using a boppy pillow that was equipped with safety straps, and the straps were not fasted. This was corrected during the inspection when the infant was removed and the primary caregiver was informed that straps must be fastened when the equipment is in use.

An infant was observed sleeping on a boppy pillow with a bottle. This was corrected during the inspection when the infant woke up and the bottle was removed.

The assistant caregiver did not have a staff file, and the primary caregiver's spouse did not have proof of a background check and orientation to the home.

Three of the children's mats were not labeled.

The operation did not have a list of employees, their most recent inspection report, or their permit renewal letter posted.

The primary caregiver only had 28 out of 30 hours of annual training, and was missing three hours of management training, six instructor-led hours, and the following taining topics: emergency preparedness, food allergies, building and physical premises safety, handling hazardous materials, and preventing communicable diseases.

A toddler did not wash their hands after a diaper change. This was corrected during the inspection when the primary caregiver was reminded that children's hands need to be washed after diaper changes, and the toddler was instructed to wash their hands.

There were two outlets without covers. This was corrected during the inspection when outlet covers were placed in the outlets.

Screen time was being used for children under the age of two. This was corrected during the inspection when the television was turned off and the primary caregiver was reminded that children under two are not allowed to have screen time.

Out of four children's records reviewed, three were missing physician information and consent to emergency treatment, and one was missing an official shot record from the physician.

The children in care were given crackers before they washed their hands. This was corrected during the inspection when the caregiver was reminded that children must wash their hands before eating and she had the children wash their hands.

There was no fire drill recorded for April. This was corrected during the inspection when a fire drill was conducted during the inspection and the children were able to exit in less than three minutes.

The primary caregiver did not have any menus available for review.

The antiseptic solution in the first aid kit had expired. The expired solution was disposed of during the inspection and the primary caregiver will purchase new antiseptic wipes.

The lunch that was served did not follow food program patterns. This was corrected during the inspection when the caregiver was informed about the components that need to be served for lunches and added additional components to the meal.

The primary caregiver is subscribed to recall emails but did not have a CPSC form verifying that she has reviewed the recall list and removed unsafe products.

There were two electrical outlets without covers. This was corrected during the inspection when the covers were placed in the outlets.

A caregiver with a provisional background check was left alone with children. This was corrected during the inspection when the primary caregiver returned.

Out of four children's records reviewed, two were missing healthcare information and emergency medical authorization, three were missing a health statement, and three were missing updated immunization records.

The primary caregiver said she does not review the CPSC product recall list and is not subscribed to the recall emails. This was corrected during the inspection when the primary caregiver was shown how to view the recall list online and subscribe to the recall emails.

The lunch that was served did not follow the pattern required by the Child and Adult Care Food Program.

The operation did not have a fire drill recorded since May 2024. This was corrected during the inspection when a fire drill was conducted and the children were able to exit within three minutes.

The operation did not have their most recent inspection report posted.

The primary caregiver did not have any annual training for the training year that was being reviewed.

During the inspection there were 9 children in care ages 21 months to 4 years old. This was corrected by three children in care leaving.

The new playground structure on the playground did not meet the required safety requirements for the tire swing, bucket swings, and rope swing. This was corrected during the inspection by the provider creating a safety plan to ensure the play structure will not be used during operation hours. The play structure will be used after hours only.

4 out 4 children files were missing the required health statements.

During the inspection it was found that the CPR certificate expired in April 2022.

2 out of the 4 files evaluated did not have documentation of immunizations.

During the inspection the operational policies did not have a policy on the promotion of outdoor play.
