During the time of the inspection, four crib mattresses were observed torn.

At the time of the inspection, staff files were incomplete and did not contain or have the following documents available for review, including attendance records, transportation safety training documentation, CPR and First Aid certificates, and affidavits. One person did not have a file at the time of the inspection.

The operation allowed a person to be present at the operation before receiving a background check determination of Eligible, Provisional Eligibility with Conditions, or Eligible with Conditions.

The operation transported a child who was not enrolled in the operation.

The operation did not follow conditions or restrictions that the CBCU placed on a person?s presence at the operation. This person is no longer employed at the operation.

The acting director is missing the controlling persons and governing body form to designate her as director.

3 classrooms were observed to have diaper changing mats with holes making the mats absorbent.

A child was served milk with a known milk allergy.

The director needs an updated CPR / First Aid training.

This standard was reviewed in part of a DFPS investigation and found to be deficient. Good judgment was not demonstrated when staff pulled a mat from under children, that were standing on them, causing an injury. Good Judgment was also not demonstrated when an incident report was completed and signed, prior to having accurate information.

A review of the Employees Who Left Employment in 2021 data from February 23, 2022, revealed that the center had not reported the number of employees who left employment in 2021 by February 22, 2022, as required.

This standard was evaluated during a DFPS investigation and found deficient. A child was injured by a caregiver at the operation on 12/21/21. The child obtained medical care. The operation did not report the incident until 12/27/21, which exceeded the 48 hour reporting time frame.

This standard was evaluated as part of a DFPS investigation and found deficient. 2 older children in care were in a nap room with 2 year old children before a caregiver came into the room. The older children are not qualified caregivers, so the group was left unsupervised until the caregiver arrived to the room.
