Based on the information received during the investigation the caregiver failed to use positive methods of discipline when trying to instruct the child to clean up toys in the classroom.

A child walked out of the building through a classroom exit leading to the parking lot while in care on 6/17/2022 and 6/20/2022. The operation didn't report the second incident on 6/20/2022 to licensing.

The director was present at the operation on June 20th prior to background results being received that afternoon.

A 10 year old child was observed in the Discovery Preschool 1 classroom with children 3 and 4 years of age. Staff did not use good judgement when placing an older child in this classroom.
