Operational policies were reviewed. The daycare didn't have listed that they have ducks at the center. Information about the ducks was added to the policies and this was corrected at inspection.

The daycare is currently providing field trips. The daycare didn't have their telephone information listed on what the children wear as required for field trips. This was corrected at inspection by printing out landyards for the children to wear on their upcoming field trip.

It was observed during today's follow up that the children were playing on an outdoor play area while chickens were walking around the play area. This was corrected at inspection by staff rounding up the chickens and moving the children to another play area.

A teacher in the toddler 1 classroom was observed across the hall at the door of another classroom leaving her class unattended.

Owner was present at the operation today after being notified that she isn't allowed to be present. The owner had to be instructed to leave the facility.

The operation isn't complying with the safety plan put in place during a current abuse and neglect investigation stating that they would not bring liquids into the classroom. Three classrooms had beverages inside the class.

The allegation is related to a DFPS abuse/neglect investigation. Based on information received during the investigation conducted by DFPS, it was determined that the operation staff waited 2 hours and 27 minutes to contact the parents regarding an injury that needed immediate medical attention.

Based on information received during the investigation conducted by DFPS, it was determined that the first aid wasn't properly rendered when a child received a serious burn injury.

Based on information received during the investigation conducted by DFPS, it was determined the incident report wasn't completed by the caregiver present at the time of the incident. The report included inaccurate information about how the incident occurred. The report also was not signed by the child's parent.

The operation still needs to provide notification to Licensing that the parents of all children enrolled at the operation have been notified regarding the safe sleep deficiency.
