At time of inspection, one caregivers file was observed to have pre-service training. FIle was missing training certificates for annual training that could not be located during inspection.

The operation was not able to inform me on how their staff will Demonstrate competency, good judgment, and self-control in the presence of children and when performing assigned responsibilities; outside of firing its staff.

The operation was unable to tell me how they will make sure that staff create an incident/ illness report any other non-routine situation that placed, or may have placed, a child at risk for injury or harm, such as forgetting a child in a center vehicle or not preventing a child from wandering away from the child-care center unsupervised outside of firing staff members

The operation was not able to inform me on how they will ensure that no child is abused, neglected, or exploited while in the care of the center; and outside of firing its staff.

The operation was not able to inform me on how they will ensure that staff understand to notify the parents of a child immediately if there is an allegation that the child has been abused, neglected, or exploited, as defined in Texas Family Code ?261.001, while in care outside of firing its staff.

Toddler rooms had a plastic sink in the kitchen area broken. (This was corrected when the caregiver threw the sink away). A mat in the library was ripped. (This was corrected when a caregiver threw the mat away). In a Pre-K room there were electrical cords hanging off of the counter. (This was corrected when a staff and a caregiver cleaned up the area and ensured the cords were not hanging.) In a Pre-K room an outlet was observed uncovered. (This was corrected when the outlet was covered). In a toddler room the carpet to a small slide was fraying at the end.A baseboard in the Pre-K Room was pealing away from the wall. A changing table was observed cracked on the edge and along the side.

During the walk through a two year old child who is medically diagnosed with Cerebral Palsy was strapped inside of a chair because the child was fighting with other children in the classroom.

Based on the evidence obtained during the DFPS investigation, the caregiver used inappropriate discipline with a child in care. The caregiver actions were concerning when it appeared that they were targeting the child's every action.

Based on the evidence obtained during the DFPS investigation, a caregiver was observed grabbing a child by the neck.

Based on the evidence obtained during the DFPS investigation, a caregiver was observed yelling at a child via video.

Based on information obtained during the DFPS investigation, a caregiver did not demonstrate good judgment when the caregiver did not tell the truth about the incident which led to the caregiver pulling a child's arm, which resulted in the child needing medical attention.

Based on the evidence obtained during the DFPS investigation, a caregiver was observed grabbing a child's right arm.

Based on information obtained during the investigation, there is sufficient evidence to support the allegation that prohibited punishment of grabbing a child was used.

Based on information obtained during the investigation, the licensing incident and illness report was not completed.

Based on external documentation, an incident report was not completed by the operation within the required timeframe. The incident report was completed 9 days later.

The operation has failed to complete the Pre-Employment affidavit on caregivers with hire date 09/18/2023 and 10/04/2023.

Four out of 10 files reviewed did not have the admission date on the form. This was corrected at inspection when the dates were added to the admission form.
