Medication was observed at time of inspection in a child's backpack on a shelf accessible to children. This was corrected at time of inspection when the medication was placed in a cabinet out of reach of children.

Loose fill on prek and school age playground was observed below six inches.

Two out of ten children's records did not have up to date immunizations avalible for review at time of inspection.

This was found to be deficient during the course of the investigation when a child's bottle was given to another child due to it not being labeled.

The operation did not report within the required two-day timeframe which includes weekends. The incident occurred on a Friday and report was made the following Monday.

It was determined through the course of the investigation that although a voicemail was left. The operation did not provide details in the voicemail on why they called and did not make further attempts to contact parent or additional parent therefore there were informed at pick-up time.

This was evaluated during the course of the investigation and determined to be deficient. A caregiver removed the tray of a highchair where an infant was not strapped in and turned away resulting in the infant falling out of the highchair.

It was determined that the highchair had been continually used without it having safety straps and therefore was not compliant. It is the responsibility of the director to ensure the operation operates in compliance.

This was evaluated during the course of the investigation and determined to be deficient. The operation was using a highchair in the infant room that did not have straps.

Verification of transportation training for both drivers was not available for review during the inspection.

A caregiver took food from one child's plate and placed it on another child's plate when the plates were already served, creating an unsanitary condition. The caregiver also took food from a plate to taste before handing it to a child.

A 4 year old who was removed from their classroom over disruptive behavior was briefly placed in the infant room with infants under 12 months old.

4 child immunization records were not current with immunization requirements.

One caregiver's CPR training was online only, which did not include use of a CPR manikin, as required.

A caregiver demonstrated lack of self-control and good judgment when redirecting a child. The caregiver lifted the child by the armpit area and then abruptly placed the child down on a cot in an inappropriate manner.

A caregiver did not give a child adequate opportunity to correct unacceptable behavior.
